Ultrasound-Guided Femoral Dialysis Access Placement

Abstract: Background and objectives: Insertion of dialysis catheters (DCs) is a prerequisite for successful initiation of hemodialysis. We attempted to determine if ultrasonography-guided (USG) insertion was superior and safer than the anatomical landmarkguided technique (ALT) for the femoral vein (FV).Design, setting, participants, & measurements: This was a randomized prospective study on 110 patients requiring FV DCs in a tertiary care hospital. Patients were randomized into two groups: USG and ALT. Data were collect… Show more

“…The overall success with the landmark method was 90.5% which is in concordance with other reports where the overall success ranges between 85% and 100% [8,17,19,21,22]. In our study, in 60.5% of the patients the catheterization was achieved in the first attempt.…”

“…Studies show increased success and reduced complication rate with the use of direct ultrasound [17,19,21]. Some studies report that two dimensional ultrasound offers minimal advantage in safety and quality during central venous catheterization of the subclavian vein (lower percent of arterial puncture and hematoma) and femoral vein (higher first pass success) [11].…”

“…Although ultrasound guidance is not essential for catheterization of central veins, it is now considered the standard of care, markedly increasing success rates and lowering complication rates (31,32). The site should be imaged before the procedure to ensure normal anatomy and adequate visualization of the target (33,34).…”

“…In a study by Hilty et al [39], which was conducted on patients who were in cardiac arrest, the superior performance of ultrasound-guided insertion was attributed to its ability to allow visualization of the vein even in the absence of a palpable femoral arterial pulse. Use of ultrasound for catheterization of the femoral artery or vein reduces vascular complications because it provides better definition of the local anatomy and decreases the chances of accidental puncture of other vessels [41,42].…”
