Ultrasound-Guided Ankle Block for Forefoot Surgery

Abstract: Ultrasound-guided ankle block is a highly effective technique for bunion surgery. The sensory territory of the SaN in the foot seems to extend only to the midfoot. According to our sample, 97% of the patients undergoing bunion surgery under an ankle block would not benefit from having a SaN block.

“…U ltrasound (US)-guided saphenous nerve block is a commonly used technique to provide complete surgical anesthesia of the foot and ankle in combination with a popliteal sciatic nerve block. 1,2 However, in part owing to its small caliber, the saphenous nerve is more difficult to reliably block than the sciatic nerve in the popliteal fossa (ie, with success rates reliably exceeding 95%). [3][4][5] As the terminal branch of the femoral nerve, the saphenous nerve provides sensory innervation to the anteromedial aspect of the lower leg, including the medial ankle joint capsule and distal tibia.…”
