Primary testicular lymphoma is rare and appears with nonspecific findings on grayscale and color Doppler sonography. We present 8 patients further examined with contrast-enhanced sonography, strain elastography, and histologic analysis after orchiectomy. Seven of 8 patients had a diagnosis of large B-cell lymphoma, and 1 of 8 had a diagnosis of granulocytic sarcoma, with solitary lesions (2 of 8), multiple lesions (3 of 8), or entire testicular involvement (3 of 8). Lesions appeared hypoechoic (7 of 8) or isoechoic (1 of 8), all with increased vascularity on color Doppler sonography and a nonbranching linear pattern of intratumoral vessels (7 of 8). Contrast-enhanced ultrasound (CEUS) confirmed this pattern and showed increased enhancement in all lesions. On strain elastography, all lesions were hard, with an elasticity score of greater than 4. Multiparametric sonography of testicular lymphoma identifies increased vascularity on color Doppler and contrast-enhanced ultrasound and increased lesion stiffness on strain elastography.

E rkek infertilitesinin en sık ve düzeltilebilir nedeni olan varikosel, pampiniform pleksusta bulunan venlerin anormal dilatasyonudur. [1] Sıklıkla sol tarafta görülür. Sol internal spermatik ven vertikal olarak seyrederek o taraf renal venine drene olur. Sol renal ven içerisindeki yüksek basınç varikosel oluşumuna yol açar. Sağ taraf internal spermatik ven tanjansiyel olarak vena kavaya açıldığı için ciddi bir basınca maruz kalmaz. [2] Varikosele bağlı infertilitenin nedeni tam olarak bilinmemesine karşın ısı artışı, artmış veya azalmış kan akımı, renal ve adrenal metabolitlerin testise reflüsü, hipoksi, sigara, hormonal bozukluk, aile öyküsü, yaş, travma, obezite, flebit ve uzun süre ayakta kalma gibi faktörlerin rol oynadığı düşünülmektedir. [3-6] Varikosel prevelansı sağlıklı erkeklerde %15-20 oranındadır. Primer infertilite nedeniyle tedavi arayan erkeklerde varikosel prevelansı %35-40 oranında iken sekonder ABSTRACT Varicocele is the most common and correctable cause of male infertility. Although the physical examination is the gold standard method in diagnosis of varicocele, some radiological imaging methods are still in use. Imaging methods used for varicocele diagnosis are venography, thermography, scintigraphy, computed tomography, and diffusion weighted magnetic resonance imaging. Up-to-date information on radiological imaging methods will be given in this review.

Introduction:The development of sonogram with high frequency linear transducer and color doppler is an important mile stone in evaluating scrotal pathologies. Computed tomography exposes testicles to the radiation and MRI is not so easily available. Therefore ultrasound with color Doppler is the best suited modality for the evaluation of scrotal pathologies as it is simple, noninvasive, reproducible, relatively inexpensive and widely available investigation that will not expose the testis to radiation. Aims and Objectives: To classify (etiologically) and to evaluate various scrotal pathologies using ultrasonography and describe the role of High resolution ultrasound and colour doppler in their diagnosis and differentiation. Materials and Methods: The study was done on 60 patients who were referred to the radiology department by the General Surgery and Urology departments for scrotal ultrasound and Doppler study. This study was conducted between Results : Of the 60 patients studied, incidence of various pathologies were as follows -inflammatory pathology in 26%, congenital in 12%, scrotal hernia in 16%, infertility in 8%, torsion in 7%, tumors in 3.3%, trauma in 3%, and others in 26.7%. The imaging features of these conditions with differentiating hallmarks are described. Conclusion:High frequency ultrasonography is invaluable in demonstrating normalcy of testes and epididymis in the presence of large hydroceles. High-frequency ultrasonography enables clear demonstration of the morphological alterations associated with acute scrotal inflammatory diseases. Color doppler sonography is highly sensitive in diagnosing acute scrotal pathology and accurately differentiates testicular ischemia /torsion from acute inflammatory conditions.
