Objective: Develop a system for pediatric peritoneal dialysis totally enclosed, easy management and assess the level of assurance of sterility.Methods: In the period February to December of 2008, was designed and developed at the Section of Bioengineering of Braile Biomédica Indústria, Comércio e Representações S/A® a system of peritoneal dialysis pediatric closed. Twenty systems were built undergoing trials on the level of assurance of sterility, and divided into group A (n=10) using the test for sterility and B (n=10) penetration of the gas ethylene oxide.Results: In group A, the sterility test was negative for the proliferation of bacteria and fungi in 14 days in all systems. In group B, the gas penetration test showed that there was penetration of gas in all points evaluated.Conclusions: It was possible to develop a new system for peritoneal dialysis pediatric closed easily managed and ensure its sterility.

Efeitos da ultrafiltração modificada na função pulmonar e necessidade de hemotransfusão em pacientes submetidos à revascularização do miocárdioThe effects of modified ultrafiltration on pulmonary function and transfusion requirements in patients undergoing coronary artery bypass graft surgery Abstract Objective: The inflammatory response after cardiac surgery increases vascular permeability leading to higher mortality and morbidity in the post operative time. The modified ultrafiltration (MUF) has shown benefits in respiratory and hemodynamic function in pediatric patients. This approach in adults is not well established yet. We hypothesize that modified ultrafiltration may improve respiratory, hemodynamic and coagulation function in adults after cardiac surgeries.Methods: A prospective randomized study was carried out with 37 patients who underwent coronary artery bypass graft surgery (CABG) were randomized either to MUF (n=20) at the end of bypass or to control (no MUF) (n=17). The anesthesia and ICU team were blinded for the group selection. The MUF were carried out for 15 minutes after the end of bypass. The patients' data were taken at beginning of anesthesia, ending of bypass, ending MUF, 24 hours, and 48 hours after surgery. For clinical outcome the pulmonary, hemodynamic and coagulation function were evaluated.Results: We observed lower drain loss in the MUF group compared to control group after 48 hours (598±123 vs. 848±455 ml; p=0.04) and required less red blood cell unit transfusions compared to control group (0.6 ± 0.6 vs.1.6±1.1 units/patient; P=0.03). The MUF group showed lower airway resistance (9.3±0.4 vs. 12.1±0.8 cmH2O.L-1s-1; P=0.04). There were no deaths in both groups.Conclusion: The MUF reduces post operatory bleeding and red blood cells unit transfusion, but with no differences on clinical outcome were observed. The routinely MUF employment was not associated with hemodynamic instability. A circulação extracorpórea (CEC) consiste na substituição temporária do coração e dos pulmões por uma máquina durante alguns procedimentos cirúrgicos. O uso de soluções cristaloides para o preenchimento e retirada de ar do circuito de CEC acarreta na hemodiluição do paciente, gerando complicações como redução dos níveis de hemoglobina e diminuição do transporte e oferta de oxigênio, havendo a necessidade de utilização de técnicas de ultrafiltração. A ultrafiltração reduz a formação de edema e necessidade de transfusões sanguíneas, além de controlar a reação inflamatória sistêmica mediante a remoção de agentes pró-inflamatórios. Na ultrafiltração com balanço zero (Z-BUF) uma solução é infundida no circuito de CEC e um volume igual de fluido é removido via coluna de ultrafiltração. Este processo permite que pequenas moléculas sejam removidas do sangue, enquanto células e grandes proteínas permanecem na circulação. A Z-BUF foi desenvolvida para reduzir mediadores inflamatórios associados à CEC, embora atualmente auxilie na correção de anormalidades eletrolíticas que ocorrem durante a cirurgia. O trabalho elucida os benefícios da Z-BUF utilizada em pacientes submetidos à cirurgia cardíaca. Observa-se que a Z-BUF ajuda na redução de fatores inflamatórios, atenua a perda sanguínea no pós-operatório e, concomitantemente, diminui o tempo de ventilação mecânica e de permanência na UTI, além de manter um equilíbrio eletrolítico mais adequado e fisiológico para o organismo.
