Ultra High-Resolution Anterior Segment Optical Coherence Tomography in the Diagnosis and Management of Ocular Surface Squamous Neoplasia

Abstract: The development of optical coherence tomography (OCT) technology has helped to usher in a new era of in vivo diagnostic imaging of the eye. The utilization of OCT for imaging of the anterior segment and ocular surface has evolved from time-domain devices to spectral-domain devices with greater penetrance and resolution, providing novel images of anterior segment pathology to assist in diagnosis and management of disease. Ocular surface squamous neoplasia (OSSN) is one such pathology that has proven demonstrabl… Show more

“…The iVue-100 SD-OCT (Optovue Inc. Fremont, CA, USA) device was used to measure CCT and RNFL thickness. The iVue-100 SD-OCT scans faster (26,000 A-scans per second) and provides higher resolution cross-sectional images (5 µm) than the TD-OCT. 5,6,7,8 RNFL measurements were performed using the optic nerve head (ONH) iVue-100 (Optovue, Inc.) protocol, which consists of 12 radial scans 3.4 mm in length and 13 concentric ring scans ranging from 1.3 to 4.9 mm. RNFL thickness was measured along a circle of 3.45 mm diameter centred on the optic disc, with the outputs from the measurements being temporal, superior, nasal, and inferior average.…”

“…5 The recently developed spectral domain (SD-OCT) technology 5 has a five times higher image resolution and 60 times faster imaging speed compared with the conventional time domain (TD-OCT) devices, resulting in improved diagnostic accuracy in detecting early glaucoma. 6,7,8 Normative databases of RNFL thickness values from age-matched normals are used to interpret thickness measurements generated by SD-OCT machines. These values vary and cannot be used interchangeably between different SD-OCT machines.…”

“…Due to the multifocal nature of OSSN, surgical excision results in extensive collateral damage to adjacent areas of normal epithelium (37) . In addition to IC, newer diagnostic techniques including CCM (22) , toluidine blue (38) , and ultra-high resolution anterior segment optical coherence tomography (UHR-OCT) (39) have been reported to aid in the diagnosis of OSSN. All these techniques have limitations and require skilled professionals to perform the tests and interpret the results.…”

“…Despite UHR-OCT having the advantage over IC of providing relatively deeper scans of the entire epithelium and the underlying tissue, it cannot reliably detect invasion (37) . In addition, UHR-OCT machines are largely limited to academic institutions (39) . IC may be an inexpensive tool that can be used in the outpatient clinic setting to help provide an objective evaluation of suspicious lesions that enables patients to make better informed decisions regarding the treatment requirements.…”

“…We used a custom-built spectral domain anterior segment UHR OCT which has a center wavelength of 840 nm, allowing for imaging with an axial resolution of approximately 3 μm [5]. UHR OCT showed marked thickening of the corneal epithelium in the area of the lesion, which is suggestive of proliferating epithelium seen in epithelial tumors such as corneal intraepithelial neoplasia and CIN [5,6]. In addition, the lesion was dense and irregular, causing shadowing posterior to the lesion (fig.…”
