Ulnar Head or Total Distal Radioulnar Joint Replacement, Isolated and Combined with Total Wrist Arthroplasty: Midterm Results

Abstract: Purpose Various implants have been described for ulnar head replacement (UHR) or for total replacement of the distal radioulnar joint (DRUJ). Many series are small and few reports on mid- or long-term results. This study is primarily aimed to report on the midterm results after ulnar head only and total DRUJ replacement using the uHead in the treatment of painful disorders of the DRUJ. The secondary aim of the study was to eventually assess the combination of UHR and total wrist arthroplasty (TWA). M… Show more

“…Results for ulnar head replacement and semi-constrained arthroplasty that replaces the whole arthritic distal radioulnar joint (DRUJ), have shown reduction in pain [1][2][3][4][5] and high patient satisfaction. [6][7][8][9][10] Ulnar head prostheses (UHP) have been shown to restore near normal biomechanics in cadaveric studies, [11][12][13] as opposed to the more traditional ulnar head resection arthroplasty, which renders the forearm unstable and biomechanically equivalent to a one bone forearm.…”

“…Noted that the uHead including its option for total DRUJ replacement was withdrawn from the the marketplace by the company stryker (USA) in 2021 (prior offered by Small Bone Innovations / USA, financial interests? ), despite well-known promising mid-term results [93]. The rationale behind this phenomenon seems to be an adjustment to the stress distribution away from the articular (sub)surface and towards to the shaft proximally which was also confirmed in a biomechanical investigation with the Maestro stem [94].…”

“…It is a radial surface replacement, thus, the minimal bony resection allows revision TWA or conversion to TWF without greater problems. Another advantage is that the distal radioulnar joint (DRUJ) is not compromised allowing an additional ulnar head replacement (UHR) [23,24]. It can be used both in a cemented or in a non-cemented manner (Figures 2A-D) [25].…”
