Ulcerative Proctitis

Abstract: Ulcerative proctitis is an idiopathic mucosal inflammatory disease involving only the rectum and is therefore an anatomically limited form of ulcerative colitis. Diagnosis is made based on clinical presentation, endoscopic appearance, and histopathology. Additionally, other etiologies of proctitis are excluded. The course of the disease is variable ranging from complete resolution to easily maintained remission to frequent relapses or refractory disease. Extension of inflammatory changes involving the proximal… Show more

“…In addition, patients with ulcerative proctitis may receive reduced or delayed benefits from surgery compared with those with more extensively-located disease. Therefore, surgery is rarely indicated for this population, while infliximab has shown efficacy in bringing clinical response in patients with ulcerative proctitis (Whitlow 2004 ; Bouguen et al 2010 ). Colectomy may thus not be a “ one size fits all ” solution.…”

“…The mainstay of treatment for ulcerative proctitis is represented by topical aminosalicilates and/or topical steroids. Oral steroids are sometimes used in case of unresponsiveness to topical therapy[ 21 ]. Moreover, it has been suggested that oral mesalamine can reduce the risk of disease extension[ 22 ].…”

“…Refractory UP is defined as more than three relapses per year, chronic disease activity on continuous medications, or need for systemic corticosteroid or immunomodulatory therapy [3]. It should be noted that the criteria for defining refractory UP are different from those pertaining to UC, since UP seldom presents with systemic features like fever or anemia.…”

“…It must be remembered however that Crohn's disease, with its cobblestoned mucosa and irregular, large and deep ulcerations can occasionally occur confined to the rectum [2], and the distinction from UP is important since the treatment and prognosis are very different. Ischemia in the rectum, radiation-induced proctitis, toxic injury, infections like cytomegalic virus and Clostridium difficile pseudomembranous colitis, parasites and sexually-transmitted diseases all enter into the differential diagnosis of UP [3]. The etiology of UP is likely the same as the etiology of UC, which is to say that it is basically unknown.…”
