The respiratory primordium buds off the primitive foregut and grows caudally, on a lengthening stalk; the cephalic end of the stalk develops into the glottis and infraglottis, and the rest becomes the trachea. Compression by pharyngeal mesoderm cephalic to the respiratory diverticulum obliterates the foregut lumen ventrodorsally as far cranially as the 4th pharyngeal pouches, forming an epithelial lamina with a narrow pharyngoglottic duct along its dorsal border. The mesoderm also raises an epiglottic and two arytenoid swellings in the pharyngeal floor at the level of the 4th pouches; the triangular ‘cecum’, bounded by these swellings, grows caudally along the ventral border of the epithelial lamina to just above the glottis. Beginning at stage 21, the epithelial lamina separates cephalocaudally, bringing the cecal lumen into continuity with the pharyngoglottic duct to form the laryngeal vestibule; when the separation is complete, the vestibule is continuous with the infraglottic cavity. Meanwhile, bilateral pouches arising from the caudal end of the cecum form the ventricles, the lower lips of which become the vocal folds, and the pharyngeal mesoderm surrounding the laryngeal cavity gives rise to the laryngeal cartilages and intrinsic musculature. The cricoid chondrifies bilaterally from a single center in the ventral arch of a precartilaginous template that encircles the infraglottic cavity, and on meeting forms the dorsal lamina. Each arytenoid chondrifies from a single center, and each half of the thyroid cartilage chondrifies from two. Anlagen for the intrinsic muscles appear during stage 17, sites of individual muscles are recognizable by stage 23, and myofibrils are pfesent by the 12th week.

In 30 human fetuses between 8 and 13 weeks of intrauterine life the lateral wall of the nasopharynx was examined by light microscopy and transmission am scanning electron microscopy. In the subjects between 8 and 9 weeks in utero the mucosa displays still an immature appearance, being mono- or bistratifiet and lacking the characteristic structures of the respiratory epithelium. Nevertheless, signs of differentiation are to be noticed, with the presence of two distinc cellular types that, in the later periods, will give rise to ciliated cells and microvillus-provided cells. An almost complete differentiation will be reached at 12–13 weeks in utero, even if goblet cells are still lacking in the examined zone during the considered period. Nonrespiratory types of epithelium, such as transitional or squamous, were never found in the studied subjects.

Μαλπιγγιακός Καρκίνος Κεφαλής Και Τραχήλου

Ο σκοπός αυτής της μελέτης ήταν να διαπιστωθεί η αποτελεσματικότητα των διάφορων θεραπευτικών επιλογών στον τοπικό έλεγχο του ακανθοκυτταρικού καρκίνου του λάρυγγα ανάλογα με το στάδιο της νόσου, την ανατομική θέση, την υποτροπή της νόσου καθώς επίσης και να μελετηθούν συγκεκριμένα επιδημιολογικά χαρακτηριστικά όπως η χρήση καπνού και αλκοόλ.Έγινε μια κλινική, αναδρομική μελέτη 164 ασθενών με μαλπιγγιακό καρκίνο του λάρυγγα, γλωττιδικής και υπεργλωττιδικής εντόπισης, όλων των κλινικών σταδίων. Σαράντα εννέα ασθενείς σταδίου Ι, πενήντα οχτώ ασθενείς σταδίου ΙΙ, πενήντα ασθενείς σταδίου ΙΙΙ και επτά ασθενείς σταδίου ΙV παρακολουθήθηκαν μετά την θεραπευτική τους αντιμετώπιση για 58 μήνες κατά μέσο όρο. Η ακτινοθεραπεία ήταν η συχνότερη θεραπευτική επιλογή στο στάδιο Ι, ενώ στα υπόλοιπα στάδια η αντιμετώπιση ήταν χειρουργική. Εξήντα πέντε ασθενείς παρουσίασαν υποτροπή, με ενδιαφέρον να παρουσιάζεται στο ποσοστό υποτροπής του πρώτου σταδίου (49%), το οποίο ήταν και το μεγαλύτερο ποσοστό υποτροπής που παρουσιάστηκε σε σχέση με τα υπόλοιπα στάδια της νόσου. Τριάντα ένα ασθενείς παρουσίασαν απομακρυσμένες μεταστάσεις και δευτεροπαθείς εντοπίσεις. Το 84.1% των ασθενών ήταν καπνιστές και το 44.5% έκαναν συχνή κατανάλωση αλκοόλ.Συμπεραίνεται ότι το είδος της θεραπείας (Χειρουργική /μη χειρουργική θεραπεία) εξαρτάται από το εάν ή όχι έχουμε υποτροπή και συγκεκριμένα η χειρουργική θεραπεία παρουσιάζει μικρότερο ποσοστό υποτροπής.Η υποτροπή δεν συσχετίζεται με τη κατανάλωση αλκοόλ αλλά ούτε και με το κάπνισμα.Η συνάρτηση επιβίωσης μεταξύ πρώτου και δευτέρου σταδίου δεν διαφέρει ενώ η επιβίωση διαφέρει μεταξύ πρώτου και τρίτου (p=0.035), πρώτου και τετάρτου (p=0.0038), δευτέρου και τετάρτου (p=0.0156) ενώ μεταξύ τρίτου και τετάρτου δεν διαπιστώθηκαν στατιστικά σημαντικές διαφορές.Η συνάρτηση επιβίωσης ανάλογα της εντόπισης δεν διαφοροποιείται.Οριακή διαφοροποίηση στη συνάρτηση επιβίωσης παρατηρείται ανάλογα της εφαρμοζόμενης θεραπείας. Διαφοροποίηση της επιβίωσης παρατηρείται ανάλογα της παρουσίας υποτροπής με διάμεση τιμή παρακολούθησης για αυτούς που υποτροπιάζουν τους 80 μήνες, ενώ αυτοί που δεν παρουσιάζουν υποτροπή έχουν διάμεση τιμή τους 173 μήνες.Η επιβίωση διαφοροποιείται ανάλογα με την κατανάλωση αλκοόλ και την χρήση καπνού.
