RESUMELe diabète sucré est une maladie chronique dont la prévalence est en constante augmentation. Il reste grevé d'un risque élevé de complications graves, essentiellement micro-et macro-vasculaires. Le diabète de type 1 est une maladie auto-immune qui conduit à la disparition des cellules B sécrétrices d'insuline et exige, dès lors, un traitement insulinique intensif optimisé. Le diabète de type 2 est une maladie polygénique dont l'expression clinique est favorisée par le mode de vie moderne, prédisposant à l'obésité. Il combine un déficit insulinosécrétoire relatif et une insulinorésistance, elle-même associée à divers facteurs de risque vasculaire. Le traitement consiste d'abord en une modification du style de vie, puis la prise de divers médicaments antidiabétiques oraux et enfin, si nécessaire, l'administration d'insuline. Une prise en charge multi-risques est indispensable pour améliorer le pronostic cardio-vasculaire. La prévention du diabète et de ses complications est un objectif majeur de santé publique.Mots clés : Diabète sucré -Complications -Physiopathologie -Prévention -Thérapeutique -Santé publique Diabetes mellitus : from clinical knowledge to public health preoccupation. SUMMARY Diabetes mellitus is a chronic disease whose prevalence is increasing worldwide. It remains associated with a high risk of severe complications, essentially micro-and macro-vascular complications. Type 1 diabetes is an auto-immune disease that leads to the destruction of insulinsecreting B cells and therefore requires an intensive optimised exogenous insulin therapy. Type 2 diabetes is a polygenic disease whose expression is favoured by inadequate lifestyle, leading to obesity. It combines a relative insulin secretory defect and insulin resistance, the latter being associated with various other cardiovascular risk factors. Treatment consists of lifestyle modifications first, then the prescription of various glucose-lowering oral drugs and finally, when requested, insulin therapy. A multi-risk intervention is mandatory to improve the cardiovascular prognosis. The prevention of diabetes and its complications is a major public health objective.
