Summary. Respiratory symptoms are common in infancy. Nevertheless, few prospective birth cohort studies have studied the epidemiology of respiratory symptoms in normal infants. The aim of this study was to prospectively obtain reliable data on incidence, severity, and determinants of common respiratory symptoms (including cough and wheeze) in normal infants and to determine factors associated with these symptoms. In a prospective population-based birth cohort, we assessed respiratory symptoms during the first year of life by weekly phone calls to the mothers. Poisson regression was used to examine the association between symptoms and various risk factors. In the first year of life, respiratory symptoms occurred in 181/195 infants (93%), more severe symptoms in 89 (46%). The average infant had respiratory symptoms for 4 weeks and 90% had symptoms for less than 12 weeks (range 0 to 23). Male sex, higher birth weight, maternal asthma, having older siblings and nursery care were associated with more, maternal hay fever with fewer respiratory symptoms. The association with prenatal maternal smoking decreased with time since birth. This study provides reliable data on the frequency of cough and wheeze during the first year of life in healthy infants; this may help in the interpretation of published hospital and community-based studies. The apparently reduced risk in children of mothers with hayfever but no asthma, and the decreasing effect of prenatal smoke exposure over time illustrate the complexity of respiratory pathology in the first year of life. Bevezetés: A bronchiolitis csecsemő-és kisdedkorban súlyos légzészavart okozhat. Leggyakoribb kórokozója a respiratory syncytial vírus (RSV). Célkitűzés és módszer: A szerzők felmérték a bronchiolitis elmúlt hat szezonbeli előfor-dulását, illetve célul tűzték ki a 2014/2015-ös járvány klinikumának vizsgálatát, továbbá az RSV-pozitív és RSV-negatív betegek csoportjainak összehasonlítását. A fertőzést RSV-gyorsteszt segítségével mutatták ki. Összehasonlították az anamnesztikus adatokat, a klinikai és laboratóriumi paramétereket. Eredmények: A szerzők osztályán a 2014/2015-ös szezonban 72 bronchiolitises beteget kezeltek (a betegek kora 4,0 ± 2,8 hónap, súlya 5622 ± 1892 gramm volt), közülük 61-nél végeztek RSV-kimutatást: 36 RSV-pozitív és 25 RSV-negatív esetük volt. A betegcsoportokban a koraszülöttek aránya jelentős (33% és 18%) volt. Az RSV-pozitív, intenzív osztályos ellátást igénylő betegek szignifikánsan hosszabb intenzív terápiát igényeltek, más statisztikai különbség nem mutatkozott. Következtetések: 2014/2015-ben a bronchiolitisszezon jelentős volt, amely tanulságként szolgálhat a 2016/2017-es szezon előtt. Respiratory Syncytial Virus (RSV) is its most frequent infective agent. Aim and method: The authors surveyed the incidence of bronchiolitis during the last 6 seasons, examined the epidemic of the 2014/15 season and compared the groups of RSV positive and negative patients. RSV was tested with a quick essay. Anamnestic data, clinical and laboratory parameters were compared. Results: 72 patients were treated with bronchiolitis. RSV test was performed in 61 cases. There were 36 RSV positive and 25 RSV negative patients. There was a high proportion of prematurely born patients (33% versus 18%, respectively). RSV positive children, who required intensive care therapy, were treated significantly longer at the ICU. No further significant difference was found. Conclusions: The 2014/15 bronchiolitis season was heavy and may be instructive before the 2016/17 season. Beside supportive care prevention (passive immunization of the risk groups) is important.
