Two‐year observational study of deferiprone in superficial siderosis

Abstract: This is the first long-term prospective study of superficial siderosis on the iron chelator, deferiprone. MRI quantification of hemosiderin appears to demonstrate a measurable reduction in half of patients and this correlated with a stabilized or improving disease course. A future placebo-controlled trial is necessary to determine whether deferiprone is an effective therapy for superficial siderosis.

“…This study increases the number of reported cases of complications related to neutropenia in patients with iSS treated with deferiprone; we are aware of only one previously reported case of agranulocytosis [6]. The largest study of deferiprone in iSS (n = 38) reported no episodes of agranulocytosis during a 2 year follow-up period [2]. Interestingly, in the previous study, patients who developed neutropenia were not re-challenged with deferiprone following discontinuation of drug due to concerns about further side effects.…”

“…Classical iSS has been previously defined as a distinct group of patients who have typical radiological features of superficial siderosis (Fig. 1) in the absence of any obvious intracranial bleeding event, and associated with a slowly progressive neurological deterioration (usually a combination of deafness, ataxia and myelopathy) [2]. It is hypothesised to result from a chronic slow and lowvolume leak of red blood cells into the subarachnoid space [3].…”

“…Findet sich keine klar definierte Ursache oder aber kann diese nicht kurativ behandelt werden, so gibt es die Möglichkeit einen Heilversuch mit einem oralen Eisen-Chelatbildner (Deferiprone) zu versuchen. Erste Arbeiten weisen daraufhin, dass es sich hier um einen erfolgsversprechenden Ansatz handeln könnte [3].…”
