Two-Staged Delayed Open Reduction and Internal Fixation of Severe Pilon Fractures

Abstract: A two-staged approach offers acceptable results for the treatment of severe pilon fractures. These results compare favorably with those of primary open reduction and of internal fixation and external fixation techniques. The major advantages include limited soft tissue complications and improved articular reconstruction.

“…Attempts at early definitive fixation of highenergy injuries have been shown to result in an increased rate of complications, including skin slough and deep infection [7,9,22,30]. Conversely, staged fracture care (provisional external fixation followed by definitive internal fixation) has been shown to result in favorable functional outcomes with decreased rates of soft tissue complications [10,18,29,34].…”

“…Tornetta used ankle sparing external fixator to allow early range of motion 4 . To avoid the complications of external fixation Sirkin et al and Patterson & Cole popularized Staged management of these fractures [5,6] . Ankle spanning fixator may be used as the definitive method to stabilize the distal tibia [7,8,9] .…”
