Two-Stage Reconstruction for Eyelid Deformities in Partial Cryptophthalmos

Abstract: The complexity of the surgical treatment increases when there is no possibility of rudimentary eye bulb preservation. In upper eyelid reconstruction, the authors adopted Mustardé's technique with fixation of the lower eyelid transposed flap to the levator muscle, leading this way to relatively good upper eyelid motility. The authors consider imperative that the upper fornix be reconstructed with adequate depth to allow perfect positioning of an aesthetic prosthesis, in this way improving the final aesthetic an… Show more

“…Five cases presented with cryptophthalmos, which was also reported in several articles. 18,19 Although bony clefts were described when the disease was first reported, 1 we only found two cases with bony clefts in our study, indicating that the bony cleft is not a presentation as common as other ocular anomalies.…”

“…4,8 Multiple techniques have been described, including Mustarde-type switch flaps, lower eyelid transposition flaps, modified Cutler-Beard full thickness flaps, preputial skin grafts, oral mucous membrane grafts, hard palate grafts, auricular polytetrafluoroethylene sheets, and amniotic membrane grafts to reconstruct the fornix. [2][3][4][5]8,9 Based on a PubMed search using the terms, "Fraser syndrome," "eyelid reconstruction," and "surgical repair," the authors were unable to find similar cases in the literature. They believe their case represents a novel technique of surgical reconstruction of incomplete cryptophthalmos in a patient with Fraser syndrome.…”

“…2 There are few reports in the literature detailing surgical approaches to managing the ocular and periocular findings in Fraser syndrome. [3][4][5] The authors present a patient with Fraser syndrome and incomplete cryptophthalmos who underwent combined evisceration with forniceal and eyelid reconstruction using the native scleral remnant. In reporting this case, compliance with Health Insurance Portability and Accountability regulations has been upheld.…”

“…Surgical steps should include dissection of corneal adhesions from keratinized cornea, mucous membrane graft, Mustarde eyelid switch flap with subsequent division, and further lower lid augmentation as required. Lessa et al, presented in 2011 present a two-stage surgical reconstruction procedure for correction of the upper eyelid and ocular bulb anomalies in cryptophthalmos (Mustardé's technique with fixation of the lower eyelid transposed flap to the levator muscle) 59 .…”
