In this report, the authors present a case of suicide committed by a 66-year-old man, inside a grave at the local cemetery, with two gunshot wounds to the head. A multiple-paged, handwritten suicide note was found in an army type bag inside the victim's car, while a second one was found in his house. Medicolegal examination of the body revealed two typical contact gunshot entrance wounds and one exit wound to the head. Toxicological analysis of the femoral blood was negative for alcohol and drugs. The history, scene and autopsy findings, along with further police investigation, indicated an obvious case of suicide. The literature is reviewed and discussed. We report this case as the place of suicide is extremely uncommon and as there is no mention of a gunshot suicide inside a grave in the current literature.

ResumoO Brasil ocupou o oitavo lugar em números absolutos de suicídios no mundo em 2012. Apesar do suicídio com arma de fogo (AF) não ser a modalidade mais comum, é frequente seu encontro na prática pericial criminal. Na maioria dos suicídios com AF há apenas um disparo efetivo e o encontro de duas ou mais lesões de entrada classifica o suicídio como atípico. Neste estudo foram avaliados os suicídios atípicos com dois ou mais disparos efetivos de AF nas necropsias do Instituto Médico Legal de Belo Horizonte realizadas entre 2006 e 2012. Foram resgatados cinco casos, correspondendo a 2,63% do total de suicídios com AF autopsiados no período estudado. A maioria dos casos era do sexo masculino, solteira, tinha pele morena, apresentava idade média de 42,3 anos e recebeu atendimento médico previamente ao óbito. Em quatro casos foram observadas duas lesões de entrada de projéteis de AF e em apenas um foram observadas três. Todos apresentavam lesões de entrada no tórax, a maioria localizada na região peitoral esquerda. Em dois indivíduos houve lesões de entrada no tórax e na cabeça. A causa da morte da maioria foi traumatismo torácico perfuro-contuso. Apesar da necropsia forense ser crucial no estabelecimento da causa médica da morte e na avaliação da capacidade de desempenh ar atos motores voluntários após um primeiro disparo, a investigação criminal de casos de suicídios atípicos é um trabalho multidisciplinar, onde cada uma das peças periciais deve se encaixar adequadamente para o estabelecimento correto da causa jurídica da morte.Palavras-Chave: Suicídio Atípico; Arma de Fogo; Necropsia Forense; Perícia Criminal. AbstractBrazil ranked eighth in absolute numbers of suicides in the world in 2012. Although firearm suicide (FS) is not the most comm on form, it is frequently encountered in crime scene investigation. In most suicides with FS there is only one effective shot and the encounter of two or more entrance wounds classifies such suicide as atypical. This study evaluated atypical suicides with two or more gunshot wounds at Belo Horizonte Medical Forensic Institute's necropsies conducted between 2006 and 2012. Five cases were examined, corresponding to 2.63% of all suicides with firearms autopsied in the period studied. The majority were male, singl e, had brown skin, with a mean age of 42.3 years and received medical care prior to death. In four cases, two entrance wounds were observed and in only one three were observed. All of them presented lesions entering the thorax, most of them located in the left pectoral region. In two individuals there were lesions entering the thorax and head. The cause of death was chest trauma for most of them. Although forensic necropsy is crucial in establishing the medical cause of death and in assessing the ability to perfor m voluntary motor acts after a first shot, criminal investigation of atypical suicide cases is a multidisciplinary work where each of the pieces of knowledge must fit adequately for the correct establishment of the manner of death.

Suicidal shots fired simultaneously to the head from two handguns are rare. The authors report about a recent case in which a Smith & Wesson cal. 9 mm pistol and a Smith & Wesson cal. .357 Magnum revolver were used. Sitting on a sofa, a 33-year-old man (member of a shooting club) fired two simultaneous shots to the head; the pistol held in the left hand was discharged into the left temple, and the revolver held in the right hand was fired into the mouth. Both weapons remained in the respective hands. An upside-down muzzle imprint in the left temporal region and recoil injuries of a mandibular incisor, and the lower lip indicated that both the pistol and the revolver had been held in an inverted manner at the time of discharge. Blood stains (backspatter) and gunshot residues were present on both firing hands, whereas forward spatter originating from the exit wounds was deposited on the wall behind the suicide's head.
