Introduction: There are numerous indications that either mannan-binding lectin (MBL) deficiency or its excessive activity are associated with adverse pregnancy outcomes. High MBL concentrations and corresponding MBL2 genotypes were shown to be associated with microvascular complications in type 1 diabetes. The aim of this study was to evaluate levels of MBL and MBL-dependent activity of the lectin pathway (LP) of complement in the course of pregnancy in diabetic mothers, based on genetic background. Materials and Methods: These parameters were determined in samples from healthy non-pregnant (control), diabetic non--pregnant, healthy pregnant, and pregnant diabetic women. Results: No significant differences in median MBL levels or LP activities were found in any study group compared to the control. However, statistically significant differences in MBL levels were noted during pregnancy between the 1st and 3rd trimesters in both healthy controls and pregnant diabetics. With regard to LP values, similar trends were evident, but statistically significant results were obtained only in the healthy pregnant group. When data analysis was confined to patients carrying the A/A (wild-type) MBL2 genotype, an increase in MBL level during pregnancy (in both healthy and diabetic pregnant women) was still observed. Similarly, LP activity increased during both healthy and diabetic pregnancies, significantly so for the former. Conclusions: Diabetes, an autoimmune disease, is a serious complication of pregnancy. Therefore, determination of MBL status might be beneficial in identifying type 1 diabetic patients who are at increased risk of developing both vascular complications and poor pregnancy outcomes.

Intrauterine infection and/or inflammatory response are evident in 60-70% of spontaneous preterm births (sPTBs). Genetic factors significantly increase this risk. however, the genetic background associated with sPTB is poorly understood. surfactant protein (sP) a, sP-D, and mannose-binding lectin (MBL) are structurally and functionally related collectins that bind pathogen-associated molecular patterns, and mostly suppress innate immune responses. RESULTS:We detected an overrepresentation of the methionine allele of the SFTPD gene (encoding sP-D) Met31Thr polymorphism in preterm infants as compared to term infants. This association was highly significant in infants of families with recurrent sPTBs (P = 0.001, odds ratio = 1.65, 95% confidence interval = 1.22-2.22); however, there was no such association with SFTPD in the mothers of these infants. Polymorphism of the genes encoding sP-a and MBL did not influence the risk of sPTB. DISCUSSION: Our results suggest that the fetal SFTPD Met31Thr polymorphism plays a significant role in genetic predisposition to sPTB. We propose that fetal immune responses influence sensitivity to preterm labor-inducing signals. METHODS: Genes encoding sP-a, sP-D, and MBL were investigated as potential candidates for association with sPTB in a population of preterm singleton infants (n = 406) and their mothers (n = 308), and in mothers with term deliveries (n = 201) and their infants (n = 201), all originating from northern Finland.

Σκοπός της παρούσας μελέτης ήταν η διερεύνηση πιθανής συσχέτισης μεταξύ των γονοτύπων που χαρακτηρίζουν την MBL ανεπάρκεια ή και αντίστοιχων χαμηλών επιπέδων της πρωτεΐνης MBL με την επίπτωση περιγεννητικών λοιμώξεων και νοσηρότητας στο ΝΤ. Επιπρόσθετα, μελετήθηκε η σχέση μεταξύ MBL ανεπάρκειας με την προωρότητα ή με άλλες μεταβλητές.ΜΕΘΟΔΟΙ Η μέτρηση της λειτουργικής πρωτεΐνης MBL στον ορό του αίματος, πραγματοποιήθηκε με την ανοσοπροσροφητική μέθοδο ELISA. Η ανίχνευση των MBL2 γονοτύπων, αναφορικά με τις γενετικές παραλλαγές στα κωδίκια 52, 54 και 57 του πρώτου εξωνίου και των πολυμορφισμών στις θέσεις -550 και -221 του υποκινητή, έγινε με πρωτόκολλο PCR-RFLP. Η επιβεβαίωση των αποτελεσμάτων έγινε με ανάλυση αλληλουχίας βάσεων. Στατιστική ανάλυση έγινε με χρήση του λογισμικού SPSS 17.0.ΑΠΟΤΕΛΕΣΜΑΤΑ. Δε διαπιστώθηκε στατιστικά σημαντική διαφορά μεταξύ των νεογνών του ΝΤ με αυτών του μαιευτηρίου, όσον αφορά τις γενετικές παραλλαγές του πρώτου εξονίου. Παρόμοιο αποτέλεσμα παρατηρήθηκε και στην περίπτωση των δυο υποομάδων των νεογνών του ΝΤ (τελειόμηνων και πρόωρων). Ωστόσο, οι σπάνιοι απλότυποι HYB, LXB και HXA παρατηρήθηκαν συχνότερα στην ομάδα των νεογνών του ΝΤ, συγκριτικά με τα νεογνά του μαιευτηρίου (p=0.009). Αν και οι γονότυποι που χαρακτηρίζουν την MBL ανεπάρκεια (XA/O και Ο/Ο) εμφανίστηκαν με υψηλότερη συχνότητα στην υποομάδα των πρόωρων νεογνών, συγκριτικά με τα τελειόμηνα, η διαφορά δεν ήταν στατιστικά σημαντική (24.5% έναντι 18.7%, p=0.393). Τα επίπεδα της πρωτεΐνης MBL ήταν σημαντικά πιο χαμηλά στα πρόωρα νεογνά, σε σχέση με τα τελειόμηνα [μέση τιμή +/-SD: 667 ng/ml έναντι 922 ng/ml, p=0.032]. Επιπρόσθετα, δεν προέκυψε συσχέτιση μεταξύ των γονοτύπων που χαρακτηρίζουν την MBL ανεπάρκεια και των αντίστοιχων χαμηλών επιπέδων της πρωτεΐνης MBL με την επίπτωση περιγεννητικών λοιμώξεων. Παρόλα αυτά, διαπιστώθηκε ότι τα νεογνά που παρουσίασαν επίπεδα συγκεντρώσεων της πρωτεΐνης MBL χαμηλότερα από 150 ng/ml αντιμετώπιζαν περίπου δυο φορές μεγαλύτερο κίνδυνο να εκδηλώσουν σύνδρομο αναπνευστικής δυσχέρειας σε σχέση με τα νεογνά που εμφάνιζαν επίπεδα μεγαλύτερα του συγκεκριμένου επιπέδου τιμών (OR:2.31, 95%CI:1.10-4.83, p=0.025). Άλλοι παράγοντες κινδύνου συνδρόμου αναπνευστικής δυσχέρειας, αποτέλεσαν η προωρότητα (OR:10.21, 95%CI:4.46-23.36, p<0.001) και οι σπασμοί (OR:21.89, 95%CI:1.80-266.148, p=0.015).ή και το περιγεννετικό stress (OR:7.08, 95%CI:2.26-22.17, p=0.001).ΣΥΜΠΕΡΑΣΜΑΤΑ Ο γονότυπος MBL2 δεν μπορεί να θεωρηθεί αποκλειστικά αντιπροσωπευτικός των επιπέδων της πρωτεΐνης MBL στα νεογνά, διότι τα επίπεδα της εξαρτώνται και από άλλους παράγοντες, με κύριο την προωρότητα. Επιπρόσθετα, σπάνιοι απλότυποι εμφανίζονται συχνότερα σε νεογνά που χρήζουν νοσηλείας στο ΝΤ, ενώ νεογνά με MBL ανεπάρκεια παρουσιάζουν αυξημένο κίνδυνο αναπνευστικής δυσχέρειας, αν και δεν παρατηρείται σε αυτά αυξημένη συχνότητα περιγεννητικών λοιμώξεων. Συμπερασματικά, τα αποτελέσματα μας αποκαλύπτουν ότι μια απλή μέτρηση των επιπέδων της πρωτεΐνης MBL, ίσως μπορεί να συμβάλει στην κατάδειξη νεογνών με αυξημένο κίνδυνο επίπτωσης ΣΑΔ.
