A bilevel positive airway pressure ventilator (BPAP) is employed as a pneumatic splint for the upper airway in obstructive sleep apnea syndrome (OSAS) treatment. In this research we developed a cascade control scheme for the BPAP ventilator using a sliding mode control, generalized super twisting algorithm (GST). The gain design and the GST algorithm analysis are done using Lyapunov functions. A mathematical model of the human respiratory system and the BPAP ventilator were developed to test the GST control, analyze, and simulate the response of a person in normal conditions, during apnea, and treated with BPAP. El ventilador por presión positiva binivel en las vías respiratorias (BPAP, por sus siglas en inglés) es usado en el tratamiento del síndrome de apnea obstructiva del sueño (SAOS). En el presente trabajo se desarrolló un esquema de control en cascada para el ventilador BPAP utilizando el control por modos deslizantes algoritmo Super Twisting Generalizado (STG). El diseño de las ganancias y análisis del algoritmo STG se realizapor medio de funciones de Lyapunov. Para probar el controlador STG, analizar y simular la respuesta de una persona en condiciones normales, durante la apnea y sometida al tratamiento BPAP se hacen uso de un modelo matemático del sistema respiratorio del ser humano y del ventilador BPAP.

This paper presents a two degree-of-freedom control method for achieving constant continuous positive airway pressure (CPAP) of an obstructive sleep apnea (OSA) treatment system. After constructing a closed-loop DC brushless motor drive together with a blower and an airway module, we develop a pressure control system for achieving CPAP control. A mathematical model of the overall open-loop system is established based on the input-output data and its system parameters are sequentially determined using the recursive least-squares (RLS) method. A two degree-of-freedom (DOF) controller, including a feedforward controller and a feedback proportional-integral controller, is proposed to maintain and follow the desired pressure setpoints. Through experimental results, the proposed control method is shown useful and effective in achieving airway pressure tracking and regulation.

Continuous Positive Airway Pressure (CPAP) ventilation remains a mainstay treatment for different respiratory disorders. Good pressure stability and pressure reduction during exhalation are of major importance condition to ensure the clinical efficacy and comfort of CPAP therapy. Obstructive Sleep Apnea (OSA) and today coronavirus (COVID-19) are the main two diseases mitigated by the CPAP. This paper introduced a systematic review of the CPAP design in terms of the hardware design, Simulation-based CPAP system, control algorithm, and the measured performance. The accuracy is used as measurement of performance and calculated from the pressure value. The accuracy was compared to the predefined U.S. Food and Drug Administration (FDA)-based threshold value in which it considers this value as a reference. The results related to the modern CPAP devices introduced in this study to explain the accuracy of experimental CPAP. These were compared with a commercial CPAP devices. Also, it was revealed how the results coincide with the error ratio defined by the FDA as an evaluation measurement. The FDA error ratio determines the performance of the optimized CPAP device. This work is the first review that presented the knowledge about engineering design of the CPAP system, so it will be the first in the literature.
