Two Cases of Food Protein Induced Enterocolitis Syndrome

Abstract

Ö ÖZ ZE ET T Besin proteini ile ilişkili enterokolit sendromu (BPİES) süt çocukluğu döneminde, her iki cinste eşit oranda görülen, dört yaşa kadar kendiliğinden gerileyen ve IgE aracılığı olmadan gelişen bir besin allerjisidir. Semptomlar besin alımından sonra kusma ve ishalle başlayıp hipovolemik şoka kadar ilerleyebilir. Genelde sorumlu tutulan besin inek sütü veya soya proteinidir. Ayrıca patates, yulaf, pirinç, tavuk ve balık gibi hipoalerjenik olarak bilinen besinler ile de hastalık tablosunun tetiklendiği gözlenmiştir. Son yıllarda hücre aracılığıyla oluşan (hücresel) besin allerjisi grubuna dâhil edilmeye başlanmıştır. Burada tipik BPİES gelişen iki olguyu, literatür ışığında tanı/tedavi yaklaşımlarımızı irdelemek ve bu konudaki farkındalığı artırmak amacıyla sunmaktayız.A An na ah ht ta ar r K Ke el li im me el le er r: : Besin aşırı duyarlılığı; enterokolit A AB BS ST TR RA AC CT T Food protein induced enterocolitis syndrome (FPIES) is a non-IgE-mediated food allergy that develops in infancy, equally common in both genders and regresses spontaneously until four years of age. Symptoms may begin with vomiting and diarrhoea following food intake and progress to hypovolemic shock. The usually accused food is cow's milk/soy protein. Disease has been reported to be triggered with potato/rice, chicken/fish and oat known as hypoallergic foods. It has recently been considered cell-mediated food allergy. Two cases of typical FPIES were presented to discuss our diagnostic and management approcahes, and to raise awareness among physicians.K Ke ey y W Wo or rd ds s: : Food hypersensitivity; enterocolitis
