Twin pregnancies conceived by assisted reproductive techniques: Maternal and neonatal outcomes

Abstract: Although twin pregnancies following IVF or GIFT are more likely to result in discordant birth weight infants, the perinatal outcome is comparable to that of spontaneously conceived twin pregnancies.

“…Studies of spontaneously conceived versus ART twins have most often shown the latter to have significantly higher rates of preterm birth, low birthweight, prematurity-associated morbidity and mortality (6,7,(9)(10)(11)(15)(16)(17)(18)(19)(20)(21), and birthweight discordancy (8,21,22). Even studies that have adjusted for monochorionic placentation and monozygosity, which are associated with growth restriction, have concluded that ART pregnancies have poorer outcomes (7,23).…”

“…(22,23) Others have found no significant differences between assisted and spontaneous pregnancies after stratification, (24) or restriction to singletons (25)(26)(27)(28)(29) or multiple gestations. (29)(30)(31)(32)(33)(34) It thus remains unclear whether the association between infertility treatments and gestational hypertension is due to biologic factors intrinsic to couples with infertility problems, to patient characteristics such as older age and primiparity, to the higher rate of multiple gestations, or to the specific infertility treatment. We therefore investigated the association between fertility treatments and gestational hypertension, with specific emphasis on the effect of different technologies and specific fertility drugs, as well as the role of multiple gestations and specific patient characteristics.…”

“…Historically, the transfer of multiple embryos led to a greater chance of pregnancy but also an increased risk of multiple birth and its associated costs, including higher risks of neonatal and obstetric complications. Elective single embryo transfer (eSET) eliminates all risk of multiple pregnancy except spontaneous identical twinning but may compromise a patient's chance of having a live birth altogether (1)(2)(3)(4).…”
