Turning 18 with congenital heart disease: prediction of infective endocarditis based on a large population

Abstract: Among young adult patients with congenital heart disease, the use of six simple clinical parameters can accurately predict patients at relatively low or high risk of IE. After confirmation in other cohorts, application of the prediction model may lead to individually tailored medical surveillance and educational counselling, thus averting IE or enabling timely detection in adult patients with congenital heart disease.

“…In adults with CHD, the overall incidence rate of IE has been reported as 11 per 10 000 person-years in the CONCOR database in the Netherlands. 21 The overall rate of IE in our study was ≈3 times lower than the published estimate in adults with CHD. The reduced frequency of IE in children versus adults with CHD parallels the trend in the general population, where the incidence of IE in adults ranges from 15 to 60 cases per million person-years, 22 whereas in children it is lower at 3.9 to 6.4 cases per million person-years.…”

“…IZW występuje rzadziej u dzieci (0,04% na rok) niż u dorosłych z CHD (0,1% na rok) [444,445]. Stwierdzany odsetek pacjentów z CHD w populacji chorych z IZW różni się znacznie (prawdopodobnie na skutek błędu wynikającego z doboru grupy) i wynosi 2-60% [446][447][448][449][450], ze zgodnie podawaną przez różne źródła niewielką przewagą mężczyzn [443,451,452].…”

“…4 However, despite these major advancements, a significant proportion of this patient population will have to undergo cardiovascular surgery at some point in adulthood, either for first corrective surgery or for reoperations to treat residual defects or long-term complications. [5][6][7][8][9] Although there has been rapid growth recently in interventional cardiology techniques to treat adults with congenital heart disease, 10 surgery remains an important treatment option, with Ϸ1 of 5 hospital admissions of adults with congenital heart disease being for cardiovascular surgery.
