Turkish Neonatal Society guideline on the management of patent ductus arteriosus in preterm infants

Abstract: Ductus arteriosus is a physiologic phenomenon in utero and it closes spontaneously in term babies. The closure is problematic in preterm infants due to the intrinsic properties of the preterm ductus arteriosus tissue. Although patent ductus arteriosus has been reported to be associated with many adverse outcomes in this population, treatment has not led to a decrease in outcomes such as bronchopulmonary dysplasia. Treatment modalities also have their own risks and restrictions. The aim of the “Turkish Neonatal… Show more

“…RDS was diagnosed based on clinical, laboratory, and radiological findings. In this study, hemodynamically PDA was considered, and the diagnosis of PDA was made based on clinical and echocardiographic findings (ratio of left atrium diameter to aortic diameter >1.4) (19). For the diagnosis of NEC, Bell's criteria were used, and any stage of NEC was recorded (20).…”

“…In the presence of hsPDA, there are different opinions about giving early medical closure treatment. The tendency to give conservative treatment has increased due to some of the reasons listed below: a) <1000g infants have 40% spontaneous closure, b) early closure treatment is related to gastrointestinal bleeding, NEC, and oliguria, c) early treatment does not cause a decrease in BPD, NEC, and neurodevelopmental morbidity (9,22). On the other hand, Klukow et al (23) in 2014, detected large PDA in 92 (56.2%) of 164 babies born under 29 GW, 44 of these babies gave indomethacin and 48 placebos.…”

“…Some guidelines are suggesting early echocardiographic imaging and early closure treatment in infants with ELBW, as well as some studies recommending follow-up symptoms without echocardiographic imaging in the first week (9,10). In this study, we aimed to investigate the necessity of performing ECHO in the first 72 hours for the diagnosis of PDA in infants with ELBW.…”

“…Either ibuprofen or paracetamol was given according to clinical findings of the infants and laboratory data. Ibuprofen was used for three consecutive days at a dose of 10 mg/kg/day on the 1st day, 5 mg/kg/day on the 2nd day, and 5 mg/kg/day on the 3rd day (21). The paracetamol treatment was administered The highest score will be given according to pH or BD.…”
