Tumour bleed manifesting as spontaneous extradural haematoma in posterior fossa

Abstract: We report a unique case of primary extradural angiosarcoma of posterior fossa manifesting as extradural haematoma in a 12-year-old boy who presented with acute onset headache, vomiting, nuchal rigidity and altered sensorium. The patient underwent a retromastoid suboccipital craniotomy on emergency basis, and the lesion was excised completely. Histopathology and immunohistochemistry revealed an angiosarcoma, following which radiation therapy was given. The patient showed complete clinical and neurological impro… Show more

“…In general, PFEDH occurs infrequently [1,2,13,14,15,16,17]. Moreover, non-traumatic PFEDH is extremely unusual, with only a few reports published in the literature [18,19,20,21,22,23,24,25].…”

“…Inherited or acquired coagulopathy is usually present in spontaneous central nervous system (CNS) bleeding episodes [26,27,28]. Other systemic pathologies, such as neoplasms, can also be observed [2,[20][21][22]. Table 1 presents the literature studies that report cases of pediatric patients with spontaneous PFEDH and associated conditions that facilitated bleeding.…”

“…[ 8 ] Children have the dura and venous sinuses as opposed to the bones, and bleeding from the venous sinus leads to progressive stripping of the dura from the bone and an expanding PFEDH. Rarely PFEDH is seen in association with other bleeding diathesis,[ 9 10 11 12 ] systemic illness,[ 13 ] after cardiac surgery,[ 14 ] and malignancies such as angiosarcoma,[ 15 ] presenting either spontaneously or after a fall.…”
