Tumoriform focal muscular degeneration in two diabetic patients

Abstract: Summary. Focal muscular degeneration in two diabetics is described. In both instances the muscular lesion was excised on the grounds of clinical suspicion of tumour. The gross and microscopical pictures agreed with those of muscular degeneration following ischaemia. Accordingly, the vascular lesions that were demonstrated, diabetic mieroangiopathy and arteriosclerosis, m a y have been of importance in the pathogenesis.Rdsumd. “…Although tissue biopsy can be used to confirm the diagnosis, it is not required for diagnosis, since MRI and a thorough clinical evaluation are generally sufficient. 7 In the case of our patient, who was being treated with immunosuppressive medication, biopsy was not pursued since the diagnosis was well supported by MRI findings, and the risk of performing a biopsy outweighed its benefits. CK levels in our patient were normal (67 U/L [normal range: 50-235 U/L]).…”

“…2 Combined characteristics of both type 1 and type 2 diabetes are exhibited in CFRD, and it is primarily caused by underlying insulin insufficiency, although insulin resistance related to acute or chronic illness may play a role. 2 First described by Angervall and Stener in 1965, 7 diabetic myonecrosis is an uncommon complication of poorly controlled DM. Since then, more than a hundred such cases have been reported.…”

“…Though this condition was first reported in 1965 [6], awareness amongst surgical healthcare professionals remains low [1][2][3]5,7,8]. Published studies discuss the difficulties in diagnosis, and also how this condition has a tendency to be under recognised and therefore sub-optimally managed [1][2][3]9].…”

“…1 They described two diabetic patients who had excision of a painful thigh mass, which was suspected of benign soft tissue tumors. Histologically they found areas of hemorrhagic necrosis surrounded by muscle that showed degenerative changes and evidence of regeneration.…”
