-Context -Gastrointestinal stromal tumors are uncommon abdominal neoplasms and can affect any portion of the gastrointestinal tract. Objectives -Describe the tomographic findings of the gastrointestinal stromal tumor of gastric origin, correlating it with the mitotic index. Methods -Twenty-one patients were selected within the period of January 2000 and 2008, with histopathological and immunohistochemical diagnosis of gastric gastrointestinal stromal tumors, who presented computed tomography done before the treatment. The tomographic variables analyzed were lesion topography, dimensions, contours, morphology, pattern and intensity enhancement through venous contrast, growth pattern, invasion of adjacent organs, presence of ulceration, fistula, calcifications, infiltration of mesenteric fat, lymphadenopathy and metastasis. The mitotic index was determined through optic microscopy, counting the number of mitosis figures in 50 high power fields. Results -The tumors were located in the body (66.7%) or gastric fundus (33.3%), with dimensions varying between 4.2 and 21.2 cm (average of 10.5 cm). The growth was predominantly extraluminal (47.6%) or intra/extra luminal (28.6%). The enhancement by venous contrast was heterogeneous in 66.7%. The statistical analysis showed that irregular morphology (P = 0.027) and infiltration of mesenteric fat (P = 0.012) presented correlation with the high mitotic index. Conclusions -In the present study, most part of the tumors were located in the gastric body, with average size of 10.5 cm, presenting central hypo dense area, heterogeneous enhancement through contrast and predominantly extra luminal growth. Irregular morphology and infiltration of mesenteric fat present statistical correlation with high mitotic level. HEADINGS -Gastrointestinal stromal tumors. Spiral computed tomography. Mitotic index.

Gastrointestinal stromal tumors (GIST) are the most common mesenchymal neoplasms of the gastrointestinal tract (1) and account for only 2% of all neoplasms of the digestive system (2). The incidence is 0.6 to 1.8 per 100,000 inhabitants. The median age at diagnosis is 63 to 67 years, and no genderrelated differences are observed. The primary origin of the tumor is most commonly the stomach (50-60%), followed by the small intestine (20-30%), large intestine (10%), esophagus (5%), and omentum, mesentery, and retroperitoneum (5%) (2). Case report A 71-year-old woman came to the emergency room for increased abdominal circumference, discomfort of 1 month's duration, and a decrease in the amount and consistency of stools, but no change in bowel movement frequency. The patient's relevant history included allergy to penicillin, hypertension, dyslipidemia, and intrinsic asthma. The physical examination revealed a large, hard, painful abdominal tumor; the laboratory workup was normal and the tumor markers were negative. The abdominal ultrasound and the abdominal and pelvic computed tomography (CT) scan showed normal adnexa, linear endometrium, and the presence of a solid mass of 17.6 x 17.6 x 11 cm. The origin could not be determined.

Introducción: Los tumores del estroma gastrointestinal (GIST) son un grupo de neoplasias poco comunes del sistema digestivo que se originan en las células intersticiales de Cajal. El artículo aborda un caso de GIST rectal de gran tamaño inicialmente diagnosticado como pólipo inflamatorio. El objetivo radica en exponer un caso que ejemplifica la dificultad diagnóstica, potencial agresivo y pronóstico adverso de los GIST. Se presenta el caso de un hombre de 68 años con GIST rectal de gran tamaño, diagnosticado finalmente en etapa IV tras una segunda biopsia. El paciente recibió derivación intestinal y cuidados paliativos, pero tuvo una evolución tórpida con metástasis y complicaciones respiratorias, falleciendo a los 2 meses. Conclusiones: El caso resalta la necesidad de más investigación sobre GIST para mejorar su detección y manejo. Amplía el conocimiento sobre su potencial metastásico, respuesta limitada a terapias y mal pronóstico en etapas tardías.
