Tumoral calcinosis: Evidence for concurrent defects in renal tubular phosphorus transport and in 1α,25-dihydroxycholecalciferol synthesis

Abstract: A 50-year-old Latin American man with tumoral calcinosis presented with hyperphosphatemia (6.62 +/- 1.04 SD mg/dl), elevated renal threshold phosphorus concentration (TmP) (7.3 mg/GFR), and 1,25-dihydroxyvitamin D [1,25-(OH)2D] (69 pg/ml) hypercalciuria (239 mg/day), and a high fractional intestinal calcium (Ca) absorption (0.74). Sodium cellulose phosphate therapy (20 g/day) lowered urinary Ca, and partially reduced serum phosphorus (P) and TmP to 5.91 +/- 0.63 mg/dl and 6.2 mg/GFR, respectively. Serum 1,25-(… Show more

“…Hyperphosphatemia has been found in some individuals, but serum calcium and parathyroid hormone levels are reported normal [3,21,22,24,26,32,34,35,38,45]. An apparent inherited disorder of phosphate and vitamin D metabolism has also been described [8,19,28,34,37,38,48]. Trauma has been implicated as a cause in selected patients [5,10,15,30,36,39,42,44].…”

“…The intra-and interassay variations were 8.5 and 15.0% respectively. Serum 25(OH) D 3 was assayed by means of a rat serum binding protein system (21,22).…”

“…Irradiation was not found to be effective. Dietary restrictions of calcium and phosphorus [18,26] or a ketogenic diet [6], if at all helpful, are unacceptable for children in their growth period. Colchicine is only feasible for a limited period of time.…”
