Tumor thrombus formation in two dogs with insulinomas

Abstract: Although gross tumor thrombus formation has been identified in humans with insulinomas, tumor thrombi have not been previously reported for dogs with insulinomas. Surgical removal of tumor thrombi via venotomy seemed to be well tolerated by the dogs. Tumor thrombus formation did not seem to adversely affect prognosis for the 2 dogs of this report.

“…Case reports highlight novel diagnostic or therapeutic approaches to dogs with neoplasia‐associated thrombosis (LOE 5, Fair) . All 4 involved carcinomas and included mammary gland carcinoma (ATE), islet cell carcinoma (tumor thrombus), adrenocortical carcinoma (external iliac venous thrombosis), and renal tubular cell carcinoma (caudal vena cava thrombosis).…”

“…Insulinomas are the most common endocrine pancreatic tumours described in companion animals and arise from the pancreatic beta cells 4 . Vascular invasion and tumour thrombus formation is rare in dogs with insulinomas, and to our knowledge has only been reported in two dogs 5 …”

“…To our knowledge, only one article has reported the presence of macroscopic vascular invasion and tumour thrombus formation in two dogs with insulinoma. In both cases, the mass was located within the body of the pancreas and resected via marginal excision, while the thrombus was removed with a cranial pancreaticoduodenal venotomy 5 . The authors of the aforementioned report commented that although en bloc resection of tumour thrombi and associated blood vessel could have been performed, resection of the cranial pancreaticoduodenal vein was not attempted because of concerns that this would have compromised the viability of the duodenum.…”
