Tumor Seeding after Robot-Assisted Radical Prostatectomy: Literature Review and Experience from a Single Institution

Abstract: Seeding of tumor cells is a rare complication of minimally invasive surgery. We reviewed and improved current knowledge of prostate cancer seeding. Materials and Methods: A literature review was performed using MEDLINEÒ, EmbaseÒ and the Cochrane Library, including cases of peritoneal and port site seeding reported after minimally invasive prostatectomy. In addition, after institutionally approved chart review a descriptive summary of a single institution experience on the topic is provided. Results: The data f… Show more

“…In castrate-resistant prostate cancer with peritoneal metastases, several treatments have been tested: chemotherapeutic agents (Docetaxel/Cabazitaxel), corticosteroids, Arbiraterone acetate or even Enzalutamide. 2 , 3 In our patient even with a neuroendocrine differentiation, the combination of Abiraterone with corticosteroids has shown earlier good results.…”

“…In the light of all these elements, one could speculate that in the presence of only peritoneal metastases, an iatrogenic seeding theory must be taken in account. 2 …”

“…The still limited spread of this technique is due to both the concerns of the use of MIS for tumors and the well-known limitations of RAS in children ( 13 , 14 ). Nevertheless, since the first cases reported by Meehan and Sandler ( 15 ), results have been encouraging.…”

“…Conventional imaging such as CT initially misdiagnosed peritoneal involvement and could thus in clinical practice result in delay in diagnosis, explaining the low frequency of nonascitic peritoneal carcinomatosis reported in literature. When analyzing the patients from Table 1 , among the ten nonascitic patients, four patients were diagnosed with modern imaging ( 68 Ga PSMA-PET, PET-Choline or MRI) [ 9 , 26 , 27 , 33 ] and four were diagnosed incidentally during surgery [ 10 , 13 , 25 , 26 ]. The increasing use of modern imaging such as 68 Ga PSMA-PET will probably allow more frequent atypical metastatic localization at early stage of development.…”
