<p>Irritable bowel syndrome (IBS) is the most commonly diagnosed gastrointestinal condition that reduces patients' quality of life. It has multifactorial etiology. Nowadays it is thought that more than one etiologic factor may contribute to heterogeneous symptoms of IBS. Crossed fused ectopic kidney is a condition where both kidneys are situated on one side and fused and drain bilaterally into the urinary bladder. Most often, it is discovered by chance when evaluating another illness. A drum-shaped bulge was discovered in the descending colon of an 18-year-old girl during the examination of a protracted period of abnormal bowel habits in the form of diarrhea, with normal overlying mucosa. On additional examination, an abdominal CT scan revealed that she had a fused left renal ectopia and an empty right renal fossa. In most of the crossed fused renal ectopia (CFRE) cases patient remains asymptomatic; in a few cases it is associated with renal agenesis, vascular malformation, incontinence, a palpable abdominal mass, urinary tract infection, high incidence of stone formation, and genital anomalies. In this case report, we emphasize on crossed fused kidney causes an external compression to the intestinal wall which may create increased visceral hypersensitivity resulting in irritable bowel syndrome with diarrhea (IBS-D) type symptoms.</p>

ObjectiveThe aim of this study was to evaluate the difference in measured glomerular filtration rate (GFR) of an ectopic pelvic kidney between anterior and posterior image processing using Gates’ method of renal dynamic imaging.MethodsA total of 10 patients were studied retrospectively, with a single ectopic kidney in the pelvic cavity and a contralateral kidney at its normal anatomical position confirmed by ultrasound, computed tomography, renal dynamic imaging, etc. All images of ectopic kidneys were processed, and GFRs were measured using anterior and posterior Gates’ method of renal dynamic imaging, respectively. The contralateral normal kidney was only processed on posterior imaging. The total GFRant of one patient, which was equal to the sum of the GFR of a normal kidney on posterior imaging and the GFR of an ectopic kidney on anterior imaging, was compared with the total GFRpost of two kidneys on posterior imaging, with the GFRtwo-sample from the two-sample method, and with the estimated GFR in the Chronic Kidney Disease Epidemiology Collaboration equation. All correlation analyses were carried out between GFRs obtained from three methods, and all patients were followed up. For statistical analysis, nonparametric rank tests were used, Bland-Altman graphs were plotted.ResultsThe mean GFR of the ectopic kidney on anterior imaging was 27.48±12.24 ml/min/1.73 m2. It was higher than the GFR (10.71±4.74 ml/min/1.73 m2) on posterior imaging (t=−2.803, P<0.05). There were statistical differences between the total GFRant and the total GFRtwo-sample (Z=−2.295, P<0.05), between the total GFRant and the total GFRpost (Z=2.599, P<0.01), between the total GFRtwo-sample and the total GFRpost (Z=−2.191, P<0.05), and between the total GFRant and the estimated glomerular filtration rate (Z=−2.803, P<0.01). The bias of the total GFRant was different from that of the total GFRpost (Z=−2.191, P<0.05). There were no differences in the precision and accuracy within 30% of the total GFRant and that of the total GFRpost (precision: F=0.351, P>0.05), but there were statistical differences in the accuracy within 10% of the total GFRant and that of the total GFRpost (P<0.05).ConclusionThe GFR measured using Gates’ method of anterior image processing was more accurate than the GFR obtained on posterior image processing in reflecting the function of an ectopic pelvic kidney in renal dynamic imaging.

ANTECEDENTES: la localización anómala del riñón en el espacio retroperitoneal se denomina ectopia renal. La ectopia renal cruzada constituye una malformación congénita excepcional con incidencia de 1 caso por cada 1350 a 1 por cada 2000 nacimientos. El tumor más común reportado en alteraciones de la fusión renal es el carcinoma de células renales. Las anomalías de la fusión renal aumentan cuatro veces el riesgo de neoplasias.CASO CLÍNICO: paciente masculino de 64 años de edad con antecedentes oncológicos de ectopia renal y pulmonar. En los estudios de rutina se detectó una masa en la fusión de ambos riñones. Se realizó istmectomía con plastia ureteral, con colgajo de Scardino y colocación de catéter doble J mediante laparoscopia asistida por robot.CONCLUSIÓN: la ectopia renal fusionada cruzada se origina por el desarrollo anormal de la yema ureteral y el blastema metanéfrico durante la cuarta y octava semanas del embarazo. Después del riñón en herradura, la ectopia renal cruzada fusionada es la anormalidad renal más frecuente. La ectopia renal cruzada es una alteración congénita poco frecuente; el cruce de izquierda a derecha es frecuente, contrario al caso aquí reportado. La relación de tumores malignos con estas anomalías representa un evento excepcional. La anatomía es impredecible, se observan anomalías vasculares en 70% de los casos lo que complica el manejo quirúrgico. Diversos estudios han intentado establecer el origen del fenotipo sarcomatoide y su relación con cualquiera de los patrones histológicos clásicos del carcinoma renal.PALABRAS CLAVE: ectopia renal cruzada, ismectomía y tumor renal.
