Esophageal cancer is currently the 8th most common cancer worldwide and the 6th leading cause of cancer-related mortality. Despite remarkable advances, the mortality for those suffering from esophageal cancer remains high, with 5-year survival rates of less than 20%. In part, because most patients present with late-stage disease, long-term survival even after resection and therapy is disappointingly low. As we will discuss in this review, multiple characteristics specific to the disease stage and patient must be considered when choosing a treatment plan. This article will summarize current standard therapies, potential application of chemoprevention drugs and the promise and partial failure of personalized medicine, as well as novel treatments addressing this disease.

BackgroundThe poor prognosis of esophageal squamous cell carcinoma (ESCC) is attributed to a high recurrence rate after surgery. Cyclooxygenase 2 (COX2) is an important regulator of cell growth, differentiation, apoptosis, and transformation. COX2 overexpression is significantly associated with the tumorigenesis and progression of diverse cancers; however, its expression and significance in ESCC remains unclear.MethodsWe enrolled 118 patients with ESCC who had undergone Ivor‐Lewis esophagectomy. The expression profile of COX2 was examined by immunohistochemistry.ResultsA high expression of COX2 correlated with a higher T staging (P = 0.014), lower differentiation degree (P = 0.002), lymph node metastasis (P = 0.009), recurrence status (P = 0.004), and tumor node metastasis (TNM) stage (P = 0.001). Cox regression analysis showed that TNM stage (P = 0.001), differentiation degree (P = 0.001), and high COX2 expression (P = 0.004) were independent risk factors of prognosis.ConclusionOur data indicated that COX2 expression level is associated with key clinicopathological features and could be an effective biomarker to predict ESCC prognosis.

Over 50% of individuals with esophageal cancer (EC) present with advanced stages of the disease; therefore, their outcome following surgery alone is poor, with only 25%–36% being alive 5 years post-surgery. Based on the evidence that the CROSS and NEOCRTEC5010 trials provided, neoadjuvant chemoradiotherapy (nCRT) is now the standard therapy for patients with locally advanced EC. However, there are still many concerning clinical questions that remain controversial such as radiation dose, appropriate patient selection, the design of the radiation field, the time interval between chemoradiotherapy (CRT) and surgery, and esophageal retention. With immune checkpoint inhibitors (ICIs) rapidly becoming a mainstay of cancer therapy, along with radiation, chemotherapy, and surgery, the combination mode of immunotherapy is also becoming a hot topic of discussion. Here, we try to provide constructive suggestions to answer the perplexing problems and clinical concerns for the progress of nCRT for EC in the future.
