Tubular ectasia of the rete testis: a diagnostic dilemma

Abstract: Tubular ectasia of the rete testis is a pathologically benign process with complex and varied aetiology. It must be differentiated from neoplastic disease of the testis clinically with patient age, mode of presentation, tumour marker status and the characteristic ultrasound and Doppler study findings. Awareness and diagnosis of this clinical entity can prevent unnecessary surgical intervention in these patients.

“…This is most often encountered as an isolated finding; however, it has been reported in association with ipsilateral renal agenesis, multicystic kidney disease, renal dysplasia, undescended testis, seminal vesicle cysts, Potter's syndrome, vesicoureteral reflux, and absence of the vas deferens. 4,5,6 The rete testis is a network of tubules carrying sperm from the seminiferous tubules to the efferent ductules, which then drain into the epididymis. Tubular ectasia is hypothesized to occur when the efferent ductules become partially or completely blocked causing spermatozoa-containing cysts to form.…”

“…Tubular ectasia is hypothesized to occur when the efferent ductules become partially or completely blocked causing spermatozoa-containing cysts to form. 5 For others, it results from failure of the mesonephric structures to fuse with the testis. The condition is benign, with no malignant potential.…”

“…Hyperuricemia is a deposit of uric acid crystals in the tubules of the epididymis that cause pain. Tubular ectasia of the rete testis (TERT) is a benign cystic condition of the testis that is mostly seen in men over 55 years of age (47). Ultrasound imaging reveals an intratesticular area containing an abundance of tight and anechoic (cystic) lesions.…”
