Despite the achieved success in the fight against tuberculosis, the disease remains an immediate problem for a number of countries including Russia. To a large extent, the reasons for the high incidence and mortality of the population are not only medical but also social in nature, which leads to the emergence of geographical patterns in the spread of the disease. The purpose of the study is to identify the spatio-temporal conditions that shape the epidemiological situation of tuberculosis in Russia at both the national and regional levels. Using GIS technologies, an analysis of the current spread of the infection in the Russian Federation was carried out based on data for the period from 2006 through 2017. Typological classification of regions according to the dynamics and magnitude of the incidence rate has been developed. Based on the cartographic analysis that was carried out, regions with the most unfavorable tuberculosis situation in the Russian Federation were identified for a more detailed study at the municipal level.

The analysis of current literature data on the study of the features of the course of free-radical reactions, as well as the state of the antioxidant defense system at socially significant infectious diseases HIV infection, hepatitis, tuberculosis was carried out. The role of this kind of reaction in the genesis and progression of socially significant infections a long time has been studied. Foreign studies of recent years have been focused on the identification of specific markers of oxidative and carbonyl stress, which make it possible to identify the redox imbalance of the cell under conditions of infection and target affect it to modulate the activity of the main transcription factors of viral proteins and the bacteria pathogenicity. Numerous sources indicate the involvement of active oxygen metabolites in a wide range of events in infected cells and tissues, including neoplastic transformation processes. These biochemical markers can be used as additional criteria for monitoring the progression of infection. At the same time, noticeable gaps in this area there are that may become the goal of future research. The issues of changing free radical reactions depending on gender, age, place of residence of patients remain practically unstudied. There is little data about intensity of oxidative stress in patients of reproductive age with HIV, hepatitis B and C, and pulmonary tuberculosis, as well as the relationship of antioxidant deficiency with reproductive disorders in conditions of infection. These data could serve as the basis for the development of pathogenetically substantiated methods for the correction of socially significant infectious diseases. Modulation of the production of reactive oxygen metabolites and oxidative stress is a potentially new pharmacological approach to reduce the effects of viral and bacterial exposure.

The study estimated the prevalence of human immunodeficiency virus infection and viral hepatitis in patients admitted to the tuberculosis hospital in 20182020. Among patients admitted for the treatment of tuberculosis, 36.8% had human immunodeficiency virus infection. Markers of viral hepatitis were also detected in 78.8% of patients with human immunodeficiency virus infection associated with tuberculosis. The most common coinfection was tuberculosis human immunodeficiency virushepatitis C, which was confirmed in 28.2% of the admitted patients. Clinical, X-ray, laboratory, and sonographic indicators were analyzed in 43 patients with tuberculosis having human immunodeficiency virus infection and viral hepatitis in various combinations. All patients were divided into three groups: group 1 included patients with tuberculosis, human immunodeficiency virus infection, and viral hepatitis, group 2 included patients with tuberculosis and human immunodeficiency virus infection, and group 3 included patients with tuberculosis and viral hepatitis. Anti-tuberculosis therapy was prescribed to all patients based on the spectrum of drug sensitivity of Mycobacterium tuberculosis, and when a human immunodeficiency virus infection was detected, antiretroviral therapy was prescribed during the first 2 weeks. Patients with viral hepatitis, if indicated, were prescribed hepatoprotective drugs. Control laboratory tests were conducted once every 2 months or more often in the presence of indications. Groups 1 and 2 were more likely to have multiple organ involvement and generalized forms, which determined the severity of the clinical course. In group 1, the activity of alanine aminotransferase and aspartate aminotransferase exceeded the normal limits upon hospital admission. Bilirubin levels were significantly higher in group 3 than in groups 1 and 2. The analysis of sonographic characteristics revealed increased size of the right liver lobe in all groups, without statistically significant differences between the groups. Adverse hepatotoxic reactions were reported in 5 (33.3%) patients of group 1, 4 (26.6%) of group 2, and 4 (30.7%) of group 3. The main manifestations of these reactions were pain in the right hypochondrium and increased activities of alanine aminotransferase and asparate aminotransferase. Patients with hepatotoxic reactions were prescribed hepatoprotective drugs, which made it possible to avoid the cancellation of complex etiotropic therapy
