Tuberculosis among Elderly Chinese in Residential Homes: Tuberculin Reactivity and Estimated Prevalence

Abstract: Objectives of this study were to determine the prevalence of positive tuberculin reactivity and associated factors among elderly nursing home residents in a population with a relatively high tuberculosis notification rate, to estimate the prevalence of active tuberculosis, and to assess tuberculin reactivity as a screening tool. A stratified, disproportional, randomized cluster sample of residents was selected and the Mantoux test (using 0.1 ml of 5 tuberculin units of purified protein derivative of tuberculin… Show more

“…The tuberculin test, despite its lack of diagnostic power, revealed that the majority (>75%) of pulmonary TB patients are reactive, regardless of age, which is in agreement with the findings of other authors, who demonstrated that up to 80% of elderly patients present sensitivity on the tuberculin test. (27) In contrast, other authors have reported that younger patients present greater sensitivity on the same test. (28) Regarding bacteriological methods, culture for mycobacteria produced positive results more often than did direct microscopy, regardless of the specimen type.…”

“…Second, underreporting or overreporting in previous years could alter the apparent trend in tuberculosis rates today. Reporting completeness in London and elsewhere in the United Kingdom may only have been around 73% in the 1980s (16)(17)(18), but has apparently improved greatly since (19). Changes in reporting completeness will lead to an apparent alteration in the epidemiology of the disease.…”

“…The high number of active TB cases for smokers as shown in both graphs in Fig. 4 may be due to the fact that inhaled smoke particles may act as a carrier for Mtb (Woo et al 1996) and that most smokers suffer from bronchitis which increases the chances of infection once the bacteria have entered the lungs ). Thus, results in Fig.…”

“…Susceptible smokers are infected with Mtb at a rate φλ T with φ > 1, since smoke inhaled by smokers itself act as a carrier of Mtb (Woo et al 1996). Susceptible smokers quit smoking at a rate α.…”
