Trocars: Site Selection, Instrumentation, and Overcoming Complications

Abstract: In recent years, laparoscopy and robot-assisted procedures have become more commonplace in urology. Incorporation of these techniques into clinical practice requires extensive knowledge of the surgical approaches and complex instrumentation unique to minimally invasive surgery. In this review, focus will be directed to laparoscopic trocars including differing subtypes, placement in select urologic procedures, and proper use with emphasis on the avoidance of complications. Differing methods for the development … Show more

“…Initial peritoneal access is achieved 2 cm inferior to the right/left costal margin in the midclavicular line, with either the blind (Verres Needle) access, with the open (Hasson) access or with the optical access trocar (1)(2)(3)5,6,21). The Verres technique implies CO 2 insufflation starting in the right/left subcostal area with a Verres needle up to 15 mmHg.…”

“…2 Some other authors also stated the necessity of using additional instruments with CT device. 9,15 For the Surgicel plug, Moazin et al 13 study which was done on 114 patients resulted in 1 case of hernia (0.8%) and 2 cases of non-infectious discharge (1.7%). 13 There are some studies which performed similar techniques to the novel Surgicel plug prior to its invention.…”

“…Urologic laparoscopic procedures have been proven to be superior to open surgery in terms of complications, convalescence, safety and efficiency, with an equivalent survival. 1 The use of laparoscopic ports or trocars is a unique component of laparoscopic surgery. The proper closure of the port site is important to prevent complications related to the port-site closure, such as port-site hernia, wound infection, wound dehiscence, and ascitic fluid leakage.…”
