Abstract:Summary
Tripod Delta (diagnostic evaluation tool for accident prevention) is a checklist-based approach to carrying out safety "health checks." This paper describes the theoretical background of the approach, which is based on a model for understanding the role of human error in accidents. The method for constructing databases from which to make checklists and use of the system to generate remedial safety plans are described. Finally, the implementation is discussed and the status is reviewed… Show more

“…Ideally, this work would involve recruiting accident investigators to use, evaluate and help refine the systemic techniques; a process that was also involved in successfully establishing other analysis methods, such as HFACS (Shappell and Wiegmann, 2000) and Tripod Delta (Hudson et al, 1994). As indicated by the data presented in Fig.…”

“…Shell International use this method in their tanker and exploration operations in the form of Tripod-DELTA (Hudson et aI., 1994). British Airways currently employ a similar method called MESH (Managing Engineering Safety Health), to identify safety occurrences in their engineering operation (Reason, 1994). Likewise Reason (1993) has produced an instrument called REVIEW, which is currently being used at British Rail.…”
scite is a Brooklyn-based organization that helps researchers better discover and understand research articles through Smart Citations–citations that display the context of the citation and describe whether the article provides supporting or contrasting evidence. scite is used by students and researchers from around the world and is funded in part by the National Science Foundation and the National Institute on Drug Abuse of the National Institutes of Health.
