Trimodal therapy for stage III-N2 non-small-cell lung carcinoma: a single center retrospective analysis

Abstract: BackgroundTreatment of locally advanced non-small-cell lung cancer is based on a combined approach. To study the impact of trimodal therapy for stage III-N2 NSCLC a single centre retrospective evaluation focusing on survival and therapy-related toxicity was performed.Methods71 patients diagnosed between March 2001 and August 2008 with pathologically confirmed stage III-N2 non-small-cell lung cancer at the University Clinic of Heidelberg were retrospectively analyzed. All patients were treated within trimodal t… Show more

“…A total of 30 studies which provided information on prognostic factors in trimodality studies using MVA (Cox proportional hazards model) were found for the interval of interest. Of these, there were two prospective phase II studies ( 16 , 40 ), five prospective studies ( 11 , 12 , 18 , 22 , 41 ), and 23 retrospective studies ( 13 15 , 20 , 21 , 23 32 , 34 , 42 48 ) (Table 1 ). Of these 30 studies, 17 were single arm studies, while the remaining 13 were two-arm studies.…”

“…If possible, a trimodal approach with chemotherapy, radiation therapy, and surgery is used in many cases to obtain optimal diseasefree and overall survival (DFS and OS, respectively) rates. Earlier studies using this combined approach have shown acceptable results with 5-year OS rates ranging from 16-38% (10,11). However, despite these curative-intent treatments, approximately 39% of all patients develop locoregional recurrence or have residual tumour within 2 years after initial treatment (12,13).…”

“…Moreover, the overall 5-year survival rate of lung cancer was about 5 % due to a late diagnosis. One third of the patients is diagnosed with stage III disease (1,2). It is reported that approximately 80% of lung cancer refers to non-small cell lung cancer (NSCLC), which includes adenocarcinoma (AC), squamous cell carcinoma (SQCC) and large cell carcinoma (LCLC) (3).…”
