Trimetazidine Revisited: A Comprehensive Review of the Pharmacological Effects and Analytical Techniques for the Determination of Trimetazidine

Abstract: Trimetazidine (TMZ) is an effective and well‐tolerated antianginal drug that possesses protective properties against ischemia‐induced heart injury. Growing interest in metabolic modulation in recent years urged an up‐to‐date review of the literature on TMZ. This review consists of two major sections: (1) comprehensive and critical information about the pharmacological effects, mechanism of action, pharmacokinetics, side effects, and current usage of TMZ, and (2) developments in analytical techniques for the de… Show more

“…TMZ as dihydrochloride has good water solubility and poor solubility in alcohol. [2] TMZ is an effective and well-tolerated anti-anginal drug that has protective properties against ischemia-induced heart injury by shifting cardiac energy metabolism from fatty acid oxidation to glucose oxidation. [3][4][5][6] It leads to the process of restoring coupling between glycolysis and carbohydrate oxidation and results in adenosine triphosphate production with a smaller amount of oxygen consumption.…”

“…Trimetazidine may affect myocardial substrate use by inhibiting oxidative phosphorylation and shifting energy production from free fatty acids (FFA) to glucose oxidation 7. It may also contribute to the preservation of intracellular levels of phosphocreatine and ATP,8 reduce calcium overload and free radical-induced injury,9 inhibit cell apoptosis10–12 and improve endothelial function 13. More recently, trimetazidine has been suggested to inhibit cardiac fibrosis through an NADPH oxidase-reactive oxygen species–connective tissue growth factor pathway14 (see figure 1).…”

“…Correcting energy insufficiency leads to reduced accumulation of sodium in cardiomyocyte cytoplasm, decreased formation of reactive oxygen species (ROS), and reduced neutrophil infiltration [5, 6]. This results in cellular membrane stabilization [7, 8]. …”
