Percutaneous coronary intervention (PCI) addresses myocardial ischaemia, but a significant subset of patients encounter major adverse cardiovascular events (MACE) post‐treatment. This meta‐analysis investigated the relationship between the post‐PCI triglyceride‐glucose (TyG) index and MACE. Comprehensive searches of the Embase, PubMed, Cochrane Library, and Web of Science databases were conducted up to 3 March 2023, using relevant keywords. The effect size was determined based on I2 statistic using random‐effects models. Cluster‐robust standard errors crafted the dose–response curve, and the GRADE Evaluation Scale was employed to rate the quality of evidence. The group with the highest TyG index had significantly higher post‐PCI MACE rates than the lowest index group, with hazard ratios (HRs) of 2.04 (95% CI 1.65–2.52; I2 = 77%). Each unit increase in TyG index corresponded to HRs of 1.82 for MACE (95% CI 1.34–2.46; I2 = 92%), 2.57 for non‐fatal MI (95% CI 1.49–4.41; I2 = 63%), and 2.06 for revascularization (95% CI 1.23–3.50; I2 = 90%). A linear relationship between TyG index and MACE risk was established (R2 = 0.6114). For all‐cause mortality, the HR was 1.93 (95% CI 1.35–2.75; I2 = 50%), indicating a higher mortality risk with elevated TyG index. The GRADE assessment yielded high certainty for non‐fatal MI but low certainty for all‐cause mortality, revascularization, and MACE. The TyG index may predict risks of post‐PCI MACE, all‐cause mortality, non‐fatal MI, and revascularization, with varied levels of certainty. A potential linear association between the TyG index and MACE post‐PCI was identified. Future research should validate these findings.

Background: Remnant cholesterol (RC) is becoming an increasingly well-recognized risk factor for cardiometabolic diseases. However, no study has explored the predictive role of RC in new-onset cancer. This study aimed to examine the associations between RC and time-weighted RC with incident cancer in the general population. Methods: This was a retrospective population-based study enrolling patients attending family medicine clinics in Hong Kong between 1st January 2000 and 31st December 2003 with at least three RC measurements during follow-up visits. The primary outcome was new-onset cancer. The secondary outcome was cancer-related mortality. Multivariable Cox regression was used to evaluate associations between baseline RC and time-weighted RC with outcomes. Results: A total of 75,342 adults (39.7% males, mean age: 62.5 years old) were included. During a median follow-up of 16.8 years, 8335 (11.1%) incident cancer and 4349 (5.7%) cancer-related deaths were observed. After adjusting for potential confounders, one mmol/L increased of time-weighted RC was associated with 41% and 62% higher risk of incident cancer (HR, 1.41; 95%CI, 1.26-1.57; p<0.0001) and cancer-related mortality (HR, 1.62; 95%CI, 1.43-1.85; p<0.0001), respectively. However, no significant association between baseline RC with risk of new-onset cancer (HR, 1.04; 95%CI, 0.82-1.31; p=0.768) and cancer-related mortality (HR, 0.85; 95%CI, 0.61-1.17; p=0.315) in the adjusted model. The association between time-weighted RC and incident cancer was significant regardless of age, gender, and remained consistent amongst those with baseline use of most cardiometabolic agents, as well as those complicated with most comorbidities. Conclusions: Higher time-weighted RC was associated with increased risk of new-onset cancer and cancer-related mortality amongst the general population.

Editorial on the Research Topic Novel phenotyping and risk stratification strategies for heart failure Since its designation as an emerging epidemic in 1997, heart failure (HF) has remained a major public health problem (1). With an estimated 64.3 million people living with HF worldwide, it is a common cause of hospitalizations and contributes significantly to healthcare costs, morbidity, and mortality (2). Although recent decades have seen massive leaps in the understanding and management of HF, much remains to be explored and the frontiers of HF research continue to be pushed, as is evident from the 15 excellent articles presented in this Research Topic.Pathophysiological understanding is critically important in all medical conditions, and HF is no exception. Here, Meng et al. presented a prospective cohort of 84 consecutive patients with acute decompensation of HF who, compared to 83 patients without HF, had higher CD4 + /CD8 + expression of T cell immunoglobulin and mucin domain-containing protein 3 (Tim-3), a unique inhibitory co-receptor expressed on the surface of immune cells and mediates immune tolerance; Tim-3 expression was also independently associated with major adverse cardiac and cerebrovascular events. These highlighted the importance of inflammation in acute decompensation of HF. Meanwhile, Li N. et al. provided a comprehensive review of the role of RNA binding motif protein 20 (RBM20) and other splicing factors in titin isoform ratio modification, myocardial stiffness, and thus pathophysiology of HF with preserved ejection fraction (HFpEF). Aside from pathophysiological insights, the authors suggested that RBM20 may be a therapeutic target for mitigating myocardial stiffness in HFpEF, which, given the relative paucity of efficacious treatments for HFpEF (3), may be an exciting prospect warranting further exploration.
