Trigeminal Neuralgia associated with Primitive Trigeminal Artery Intermediate Type : Case Report

Abstract: 三叉神経痛(trigeminal neuralgia:TN)は,三叉神経 の root entry zoon(REZ)における脳血管の圧迫が主な 原因といわれている.主な責任血管としては屈曲した superior cerebellar artery(SCA) ,anterior inferior cerebellar artery(AICA) ,basilar artery やその分岐動脈,静脈 が知られている.遺残三叉神経動脈 (primitive trigeminal artery:PTA)やその変種(PTA variants)が TN の責任 血管となる割合は 0.2~0.6%とまれである 5) .さらに, PTA 本幹から小脳の皮質動脈が分岐するパターンである PTA intermediate type が TN の原因となった報告は過去 に 2 例のみである 4) 10) . 今回われわれは PTA 本幹から分岐した anomalous artery が小脳橋角部にて三叉神経を圧迫することで生じ た TN の 1 例を経験した.術前の 3D MRI/MRA fusion 連絡先:山口大志,〒 060-85… Show more
