Trigeminal neuralgia and other neuropathic pain syndromes of the head and face

Abstract: Trigeminal neuralgia is the most common craniofacial pain syndrome of neuropathic origin. Although the diagnosis remains based exclusively on history and symptomatology, modern diagnostic techniques, particularly high-resolution magnetic resonance imaging, provides valuable new insight into the pathophysiology of these cases with additional implications for therapeutic strategies. Other neuropathic syndromes affect the trigeminal nerve and warrant different treatments with varied rates of success. Rarely, neur… Show more

“…The description of the pain can be identical to that seen in primary TN, with the exception that it tends to be accompanied by a continuous dull pain in the background and by trigeminal hypoesthesia or depression of the corneal reflex [2]. The age of onset may be somewhat younger than in the idiopathic cases, but it depends on the type of the structural lesion.…”

“…Tricyclic antidepressants, gabapentin, intrathecally local anaesthetics and corticosteroids are the medical treatments used. In the surgical approach, peripheral neurectomy, percutaneous gangliolysis, motor cortex stimulation, stereotactic trigeminal nucleotomy, stimulation of the Gasserian ganglion and trigeminal rootlets are used for trigeminal neuropathic pain (3,12,13,16,18,20,21,26,27). However, we performed CT-guided trigeminal tractotomy for the treatment of this special case.…”

“…Also how to manipulate such a problem becomes more contentious in cases with bilateral complaints. A multidisciplinary evaluation is essential for this population (11). Even controversial, recommended analgesics and other pain modulators may fail to achieve pain relief.…”
