Trends in United States blood collection and transfusion: results from the 2013 AABB Blood Collection, Utilization, and Patient Blood Management Survey

Abstract: Compared to 2011, WB and RBC collections declined significantly in 2013 and disproportionately to the significant reductions in WB and RBC transfusions. Distributions of PLTs and plasma for transfusion declined in 2013, as did transfusions of plasma, while transfusion of PLTs increased significantly. Decreases in outdated and wasted components by hospitals suggest improvements in product and inventory management. Ongoing national surveys allow for trend analysis and are important for future planning.

“…Instead, the availability of blood products fluctuates, and shortages, if they exist, often vary by region and by the time of the year. [10][11][12][13][14] However, among men who are interested in donating blood, 87.1% believed that there is a national blood shortage. These data demonstrate that the public is likely unaware of the current blood supply and blood utilization programs.…”

“…In 2013, 1.2 million apheresis collection procedures yielded 2.2 million apheresis units in the United States. 10 If all collections were optimized to meet the current IBS FDA-approved guard-bands using our data, then ;300 000 products would be lost and 600 000 IBS products would be available. With a national split rate of 1.8, then 160 000 more collections would need to be performed.…”

“…gynecologic and obstetric procedures, heart valves, spine and colon surgery) have been added. In 2015, RBC usage in Ontario was 27.4 units per thousand population, among the lowest internationally [72]; by contrast, data collected in an AABB survey showed that RBC usage in 2013 in the USA was 40.3 units per thousand population [73]. The ONTraC TPs also emphasize the need for tolerance of anemia and 'restrictive' transfusion thresholds when appropriate and, in Ontario, for knee arthroplasty for example, whereas in 2002 the mean nadir Hb (transfusion trigger surrogate) was 84 g/L, it has decreased to 72 g/L.…”
