CC-chemokine ligand18 (CCL18) ismainly expressed by alternativelyactivated macrophages and DCs and plays an important role in lung fibrosis, arthritis and other diseases. Here CCL18 was measured in sera of 31 healthy volunteers and 170 patients with lung cancer and correlatedthese data with histology, tumor stage and clinicalparameters. Mean CCL18 serum levelofthe patients with non-small-cell lung cancer was 150(857) ng/ml vs. 32(61) ng/ml in the healthy control group. Patient groups differ significantly according their histology (adenocarcinoma 143(528) ng/ml vssquamous cell carcinoma 187(857) ng/ml, p,0.02). In addition, we found a significant difference between patients with lower versus higher T-stage (p,0.003). Receiver operating characteristic (ROC) analyses revealed a cutoff point of 83 ng/ml (area under the curve (AUC): 0.968; p,0.0001) to discriminate between healthy controls and non-small-cell lung cancer patients. ROC analyses to discriminate between patients, who died because of cancer related death and those who died for other reasons did not lead to a valid AUC. To stratify the tumor patients, a criterion value plot was performed leading to a point of equal sensitivity and specificity (54%) of 162 ng/ml. Patients with a CCL18 serum level higher than 160 ng/ml had a mean survival time of 623 days. In contrast, those in patients with a baseline level between 83 ng/ml and 160 ng/ml the mean survival time was 984 days (p,0.005). Survival-analysis revealed in adenocarcinoma a mean survival of 1152 days in the group below 83 ng/ml. In the median group the mean survival time was 788 days and in the group with the highest levels the mean survival time was 388 days (p,0.001). In contrast, we found no correlation between the FEV1 and the CCL18 baseline level. In conclusion, in patients suffering from adenocarcinoma increased serum CCL18 levels predict a diminished survival time.

Lung cancer is the leading cause of cancer-related death and is usually diagnosed at an advanced stage, where lymph node/distant metastases are present. Only in about one-third of patients with non-small cell lung cancer (NSCLC) the disease is diagnosed as localized, for which a complete resection is the treatment of choice and a cure is possible. Hence, in most cases systemic therapy is required, either as part of a multimodal treatment or as sole therapeutic option. Despite advances in individualized approaches and modern chemotherapeutic regimens, the course of the disease remains unfavorable (1-3).It is widely acknowledged that the tumor microenvironment plays a critical role in tumor growth and spread, and in recent years, multiple therapeutic approaches have been developed based on this knowledge (4). In contrast to agents focusing on molecular targets such as the epidermal growth factor receptor (EGFR), and anaplastic lymphoma kinase (ALK), whose activity is confined to tumors with specific genetic alterations, immunotherapy is effective in various histological subtypes (5-8).Tumor microenvironment consists of tumor cells, the leukocyte infiltrate and the tumor associated immunologic microenvironment. One of the key players, the tumor associated macrophages (TAMs) can make up a large fraction of the tumor mass (9). Depending on the tumor microenvironment, macrophages can be polarized into different functional states, of which the two extremes are known as the M1 and M2 phenotypes. M1 macrophages have been shown to carry out pro-inflammatory, cytotoxic and thus mostly anti-neoplastic effects, whilst the M2 phenotype is responsible for tissue remodeling and repair and is believed to be involved in the subversion of adaptive immunity and thus the immunological interactions that promote tumor growth and progression. TAMs in the tumor microenvironment are mostly found to have the M2 phenotype (10). TAMs secrete a specific pattern of cytokines and chemokines and are one of the main producers of CC-chemokine ligand 18 (CCL18) in this context (11,12). CCL18 is responsible for the recruitment of naive T-cells and dendritic cells, and is capable of inducing regulatory T-cells. In the tumor microenvironment, it has been demonstrated to promote tumor spread and is believed to be one of the factors involved in the immune escape of tumor cells (13)(14)(15)(16)(17). 4667

Abstract. Paclitaxel (PTX)-based chemotherapy is a standard treatment for human lung adenocarcinoma, but treatment often fails since resistance develops. Recent studies have described the activity of long non-coding RNAs (lncRNAs) in many biological processes and human diseases. Chemotherapy resistance is one of these areas, but the role of lncRNAs in paclitaxel resistance of human lung adenocarcinoma cells has not been reported. A paclitaxel resistance model was established using A549 human lung adenocarcinoma cells. lncRNAs and mRNAs were profiled in parental A549 and paclitaxel-resistant A549/PTX cells by microarray analysis. Real-time quantitative PCR (RT-qPCR) was used to validate the results of the microarray. Chromosomal distribution patterns of differentially expressed lncRNAs and mRNAs were assessed. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analyses were performed using gene set enrichment. We screened 1,154 lncRNAs and 1,733 mRNAs that had a >3-fold difference in expression in A549/PTX cells compared with A549 cells, most of which were downregulated. Nine lncRNAs and six mRNAs were randomly selected and validated by RT-qPCR. Most aberrantly expressed lncRNAs and mRNAs were located on chromosomes 1, 2, 6, 12 and 17, particularly on chromosome 1. Bioinformatics, GO and KEGG pathway analyses, revealed that some differentially expressed genes regulated classical functions and pathways such as cytosol components, protein binding, gene expression and metabolic pathways. Differential expression of lncRNAs and mRNAs in A549/PTX and A549 cells indicates that various lncRNAs may be useful diagnostic or prognostic markers of resistance to treatment, or future targets for paclitaxel-based chemotherapy, providing a novel rationale for clinical treatment.

Abstract: Our study showed a recent and significant stage migration in patients with NSCLC. It is likely that increased acceptance and widespread use of (18)fluorodeoxyglucose-positron emission tomography scan and routine brain imaging could account for these changes.
