Trends in provider-initiated versus spontaneous preterm deliveries, 2004–2013

Abstract: Objectives 1) To estimate the proportion of preterm deliveries at a tertiary perinatal center that were provider-initiated vs. spontaneous before and after a 2009 policy to reduce elective early-term deliveries. 2) To evaluate if shifts in type of preterm delivery varied by race/ethnicity. Methods We performed a retrospective cohort study of preterm deliveries over a 10-year period, 2004–2013, including detailed review of 929 of 5,566 preterm deliveries to designate each delivery as provider-initiated or spo… Show more

“…However, the preterm births were not classified into subtypes i.e., spontaneous versus indicated [36], and more than half of the preterm birth babies were delivered via CS (55.9%). This is in line with the trend of increasing indicated preterm deliveries over spontaneous ones [37].…”

“…Unfortunately, various studies have documented an epidemic of CS which has been reported to have adverse pregnancy outcomes, such as preterm birth and LBW [35][36][37][38]. Among preterm births and LBW that were delivered by CS, more than half of those deliveries were planned.…”

“…This nding resonates with a studies conducted in the United Arab Emirates (27) and China (32). Some studies have reported an epidemic of CS which these studies have found to increase adverse birth outcomes such as LBW and preterm delivery (33,34). A plausible explanation for this nding in this study could be attributed to the abuse of planned CS.…”

“…4 Despite the worldwide increase in iatrogenic preterm births, a recent study from the United States found a decrease in the overall preterm birth rate due to a decrease in the iatrogenic preterm births following the guideline changes in 2007 and 2011. 22 The rate of preterm births in the United States decreased from 12.3% to 11.2% from 2003 to 2013 and the proportion of iatrogenic preterm births decreased from 48.3% to 41.8%, 22 although the preterm birth rate again increased from 2014 to 2017. 25 The increase of iatrogenic preterm births in our study was significant even after adjusting our regression models for the identified medical indications.…”
