Trends in Low-Value Carotid Imaging in the Veterans Health Administration From 2007 to 2016

Abstract: Key Points Question How has utilization of low-value carotid imaging for carotid bruits, preoperative evaluation, and syncope changed in the Veterans Health Administration system from 2007 to 2016? Findings In this serial cross-sectional study of 809 071 carotid imaging examinations in the national Veterans Affairs Health system, there was a modest decrease in rates of imaging for carotid bruits that predated Choosing Wisely recommendations, and no signific… Show more

“…The following were the most frequently reported low-value examinations: imaging in low back pain [ 121 131 ] and knee MRI without red flags [ 121 , 141 147 ], staging and follow-up in several types of cancer (X-ray, CT, MRI and nuclear medicine) [ 58 , 116 , 172 , 199 203 , 209 , 234 262 ], abdominal CT in self-limiting episode of suspected urolithiasis [ 190 , 204 206 ], chest CTA [ 210 216 ] and ultrasound lower limb veins in patients with low risk of thrombosis [ 217 221 ] were most prominent among adult populations. When analyzing the extent in use of low-value imaging additional examinations were identified; low interval DEXA screening, echocardiography in patients with low risk of cardiac disease, carotid imaging in syncope, X-ray in rhinosinusitis, and MRI for pain in the hip or upper extremities [ 140 , 157 159 , 173 , 277 , 365 , 366 , 369 , 373 , 376 ].…”

“…Prescription habits following publication of guidelines are mainly influenced if supported by multimodal interventions including educational material and awareness campaign and it often takes years for them to shift practice. With awareness campaigns we observed some temporary improvements in mitigating overtreatment 16 , 17 , however, these measures appear insufficient in creating a lasting change 18 , 19 . Following the release of the SSGIM recommendations on RBCTs in May 2016, awareness on the new guidelines was promoted within our hospital network mainly within the department of internal medicine, but not other specialties.…”

“…Nineteen studies focused on the CW initiative and one study 34 focused on the NICE reminders. Evidence-based recommendations from wide-ranging specialties were explored, including cardiology, 30,44,45 geriatrics, 31 gynecology, 34 hematology, 35 nephrology, 30,45 obstetrics, 34 oncology, 36,38,40,42,43,46,47,49 primary care, 30,37,41,45 radiology, 31,39,45 and urology. 33,34,48 Most studies only targeted one recommendation of a specialty, with 3 studies 30,34,45 investigated multiple recommendations across different specialties and another 3 studies 37,42,43 explored various recommendations within one specialty.…”
