Trends in Engagement in Advance Care Planning Behaviors and the Role of Socioeconomic Status

Abstract: We investigated the trends in advance care planning (ACP) between 2002 and 2010 and whether socioeconomic status explained such trends. We conducted a pooled regression analysis of Health and Retirement Study data from 6052 proxies of deceased individuals. We studied 3 ACP behaviors, discussing end-of-life (EOL) care preferences, providing written EOL care instructions, and appointing a durable power of attorney for health care (DPAHC). ACP increased by 12% to 23% every 2 years from 2002 to 2010. Higher househ… Show more

“…Although some studies have reported significant associations, 8,34,35 others have not. 36 If there is an association between education and completion of ADs in patients with advanced cancer, it was not detectable in this sample. Some research suggests that the effect of SES on future planning is primarily due to income 17 and health literacy, 37 rather than education.…”

“…14 However, other work that used education and income as proxies for SES and examined the trends over time demonstrated that SES had a limited role in advance care planning although those with a higher SES were more likely to have a designated power of attorney. 15 Deborah Carr’s work examined assets, occupation, and education and found that although all of these SES measures were associated with rates of advance care planning (as SES increased, ACP increased), assets were the most powerful predictor. Assets also demonstrated a graded association with increased ACP as assets increased, a relationship not seen with education and occupation.…”

“…15 Sociodemographic factors that influence ACP have also been explored, with wide variation by race/ethnicity, 16 gender, 17 and education level. 18 However, the socioeconomic factors that may affect ACP are relatively unexplored in the literature, despite the association of socioeconomic status with respect to nearly every health-related outcome, ranging from quality of care to mortality. 19 Furthermore, there is a gap in the literature on the contextual forces that influence ACP.…”
