Trends in Cesarean Delivery: Rate and Indications

Background: The indications of Caesarean Section (CS) have been changing over time, along with a visible, much talked about rise in the rate. Without a doubt, C-sections can effectively prevent maternal and perinatal mortality when done for a medical indication. However, little is researched about the extent of maternal indications and the fetal outcomes of C-sections.Methods: Data of January to March 2012 and January to March 2017 was obtained by detailed study of the patient files from the Medical Records Department of the hospital. A predesigned proforma was used to record the relevant information. Data was analysed using standard statistical methods.Results: Cephalo pelvic disproportion (CPD) and fetal distress (FD) remained the major causes of CS in both the groups, being 50.4% in group2012 and 60.5% in group2017. Trends of CS due to maternal demand and tubal ligations are emerging. Though the rise in CS was not significant in the 5 year period having gone up from 45.41% to 48.81%, there was significant increase in deliveries without complications, having jumped up from 74.8% to 88.3%.Conclusions: As per the study, there was non-significant rise in the incidence of C-sections in the time period of study. Major contributory factors observed were CPD and FD, in which it is imperative to perform C-sections.

Primary cesarean section in multigravidae: a case control study

Background: Primary Cesarean Section (CS) is defined as CS in a patient who has not had a prior CS. While primigravidae contribute to the bulk of those undergoing primary CS, multigravidae (with only prior vaginal delivery) can also contribute. The purpose of this article is to examine the contribution of the group of multigravidae (with only prior vaginal delivery) and how they differ from primigravidae.Methods: A review of records was conducted for one year period. The confinements which were first or second para (which had undergone CS in either index pregnancy or previous pregnancy) were divided into primipara: primary CS and second para: primary CS. Various parameters were compared between the primipara undergoing primary CS and multipara undergoing primary CS.Results: Out of 1154 births, the number of CS was 401. Primary CS in primiparas was 266 (66.3%) and primary CS in multipara (with only prior vaginal deliveries) was 61(15.2%). Booking status, baby weights and NICU admission rates did not different significantly. Preterm deliveries, higher number of prior abortions; malpresentations, deep transverse arrest, antepartum hemorrhage was more common among the multipara group and the difference was statistically significant (p value < 0.05).Conclusions: Multipara (with prior vaginal delivery only) definitely contribute to primary CS rate and cannot be neglected. The profile of patients and the indications of CS in multipara appear to differ from those in primipara.

Prevalence and determinants of caesarean section in a rural tertiary teaching hospital: a 6-year retrospective study

Background: Caesarean section is the most commonly performed surgery in obstetrics and there is a rise in caesarean section rates in recent times. This study was undertaken to know the changing trends in caesarean section rate in a rural hospital and to examine the indications contributing to it. The objective of the present study was to know the prevalence and changing trends in caesarean section over the last 6 years (April 2012-March 2018).Methods: Demographic data for all the deliveries, mode of delivery and indications of caesarean sections performed from April 2012 to March 2018 that occurred at MIMS were collected in a retrospective manner.Results: Among a total of 12,522 women delivered during the study period of 6 years, 44.93%, 48.49%, 48.41%, 50.9%, 45.48% and 49.62% were delivered by caesarean section during 2012-2013, 2013-2014, 2014-2015, 2015-2016, 2016-2017 and 2017-2018 respectively. Increase in repeat caesarean section is the primary reason for these increased rates followed by fetal distress. There is a rise in the repeat caesarean section from 44.36% in 2012-2013 to 55.67% in 2016-2017 and 47% in 2017-2018. Whereas primary caesarean section rate reduced from 55.63% in 2012-2013 to 44.32% in 2016-2017 and 53% in 2017-2018.Conclusions: As repeat caesarean section and fetal distress are the most common causes of caesarean section we need to address these to bring down the caesarean section rate.
