Trends and Utilization of Laser Prostatectomy in Ambulatory Surgical Procedures for the Treatment of Benign Prostatic Hyperplasia in New York State (2000–2011)

Abstract: TURP remains the most common procedure. However, the rate of use has declined over time. In contrast, laser use has significantly increased. Laser treatment was utilized more in younger patients, in those privately insured, in hospitals with high volumes of BPH procedures, and in patients with fewer comorbid conditions. Both surgeries are safe with no differences in terms of occurrences of morbidity and complications.

“…In contrast, Pierre et al reported surgical procedures in New York State from 2000 through 2011. 5 Although the number of procedures decreased until 2008, similarly to Malaeb's study, after 2009 the surgical procedures began to increase with the increase of laser treatment. A Canadian study in 2013 by Hueber et al also showed an overall progressively growing trend of laser procedure alternatives to TURP.…”

“…However, in the last two decades there has been a large increase in treatment options. This has mainly been provided by the increase in laser surgery techniques developed (3). Holmium laser enucleation of the prostate (HoLEP) is one of these procedures, first described in its current form by Fraundorfer, and Gilling in 1998 as a natural progression from the holmium laser resection of the prostate (HoLRP) previously developed (4,5).…”

“…Parallel to the technological developments, laparoscopic/robotic simple prostatectomy and laser enucleation techniques are applied with lower perioperative morbidity compared to OSP in the surgical treatment of large prostates [11 -14]. This situa- Fisher's exact test tion caused a decrease in the popularity of OSP today [15,16]. On the other hand, in the surgical treatment of patients with large prostates, OSP remains a proven approach applied by urologists who cannot benefit from all these technological developments.…”
