Trends and outcomes of fungal infections in hospitalized patients of inflammatory bowel disease: a nationwide analysis

Abstract: Background: Immunosuppressive therapy is being increasingly used in the management of inflammatory bowel disease (IBD) which comprises of ulcerative colitis (UC) and Crohn's disease (CD). Patients on immunosuppressive therapy are at increased risk of developing opportunistic fungal infections. We conducted this analysis to describe the epidemiology of opportunistic fungal infections in this cohort. Methods: We analyzed the National Inpatient Sample (NIS) database for all subjects with discharge diagnosis of IB… Show more

“…C. tropicalis and C. glabrata were reported in 2.6% (2/78) of symptomatic patients with moderate to severe ulcerative colitis treated with infliximab. This low frequency was in line with that reported by Mushtaq et al (2%) [45]. They may exacerbate colitis progression via increased intestinal permeability and modulated host purine metabolism [46].…”

“…Candida species were the most common opportunistic fungal infections in the gut of IBD patients receiving biological therapy with high mortality [45,46]. C. tropicalis and C. glabrata were reported in 2.6% (2/78) of symptomatic patients with moderate to severe ulcerative colitis treated with infliximab.…”

“…For in-patients with IBD an overall incidence for histoplasmosis, pneumocystis, cryptococcosis, aspergillosis, blastomycosis, candidiasis, and Coccidiomycosis has been reported between 1.8 and 3.1%. Mortality in patients with IBD was up to 1.5% and with IBD and fungal infection 5.6% [39 ▪ ].…”

“…IBD can be treated with immunomodulators such as steroids and azathioprine, and biologics such as anti-tumor necrosis factor (TNF) and anti-integrin monoclonal antibodies. However, anti-TNF treatment can lead to opportunistic infections (such as bacterial, fungal, and viral infections) (Table 1), and the danger is more significant when it is used with steroids [15]. In this article, we look at viruses that play a role in the onset or progression of IBD.…”
