Pneumonia caused more than 2,56 million deaths in 2017 worldwide. With the high number of pneumonia sufferers, the use of antibiotics will also increase. This study aims to describe the quantity of antibiotic use in pneumonia patients using the Systematic Literature Review method. The literature search was conducted through the PubMed, Sciencedirect, Mendeley, BMC, and Google Scholar databases. The literature used is a journal of the last 10 years that discusses the use of antibiotics in pneumonia patients calculated by the ATC/DDD system. Of the total 19 journals included in the study, 8 journals were from Indonesia. A total of 5 journals discussed antibiotics used in CAP (Community-Acquired Pneumonia), 2 journals in HAP (Hospital-Acquired Pneumonia), 2 journals in VAP (Ventilator-Associated Pneumonia), 2 journals were non-specific, and the remainder covered several types of pneumonia. The class of antibiotics with the highest DDD value is fluoroquinolones, with an average use of 41,81 DDD/100 patient-days; 17,29 DDD/100 bed days; and 0,0696 DDD/1000 inhabitants per day, and third-generation cephalosporins, with an average use of 15,86 DDD/100 patient-days and 28,31 DDD/100 bed days. The antibiotic with the highest DDD value was levofloxacin, with an average use of 31,29 DDD/100 patient-days; 12,22 DDD/100 bed days; and 0,0612 DDD/1000 inhabitants per day, and ceftriaxone, with an average use of 12,51 DDD/100 patient-days and 26,33 DDD/100 bed days. The most widely used antibiotics in the treatment of CAP and HAP are third-generation cephalosporins, while penicillin is the most commonly used in VAP.

Purpose We aimed to investigate the impact of enhanced in-hospital infection prevention during the coronavirus disease 2019 (COVID-19) pandemic on postoperative pneumonia in older surgical patients. Patients and Methods We retrospectively reviewed the electronic medical records of consecutive patients ≥70 years who underwent elective surgery between 2017 and 2021 at our institution. All perioperative variables were retrieved from the electronic medical records. The primary outcome was new-onset postoperative pneumonia during the hospitalization period. Since February 2020, our institution implemented a series of policies to enhance infection prevention, hence patients were divided into groups according to whether they underwent surgery before or during the COVID-19 pandemic. An interrupted time series analysis was performed to evaluate the difference between pre- and post-intervention slopes of the primary outcome. Results Among the 29,387 patients included in the study, 10,547 patients underwent surgery during the COVID-19 pandemic. Although there was a decreasing trend of the monthly incidence rate of postoperative pneumonia compared to before the COVID-19 pandemic, there was no statistical significance in the trend (slope before COVID-19 period: β-coefficient, −0.007; 95% CI, −0.022 to 0.007). Conclusion Our study revealed that enhanced in-hospital infection prevention implemented to manage the COVID-19 pandemic did not significantly affect the decreasing trend of postoperative pneumonia at our institution.

Background Pneumonia, which is the third leading cause of death in South Korea, is continuously increasing with the aging society. The Health Insurance Review and Assessment of South Korea conducted a quality assessment (QA) for improving the outcome of community-acquired pneumonia (CAP). Methods We conducted a nationwide cross-sectional study of hospitalized CAP in South Korea. First to third QA data were gathered into a single database. The national health insurance database was merged with the QA database for analyzing the medical claims data. Comorbidities, pneumonia severity, and pneumonia care appropriateness were calculated using Charlson comorbidity index (CCI), CURB-65, and core assessment of CAP scores (CAP scores), respectively. Results Overall, 54,307 patients were enrolled. The CAP scores significantly improved on QA program implementation ( P < 0.001). All the variables demonstrated an association with in-hospital mortality, hospital length of stay (LOS), and 30-day mortality in the univariate analyses. Following the adjustments, higher CCI and CURB-65 scores were associated with higher in-hospital mortality, longer hospital LOS, and higher 30-day mortality. Male sex was associated with higher in-hospital/30-day mortality and shorter hospital LOS. Higher CAP scores were associated with shorter hospital LOS ( P < 0.001). Upon QA program implementation, in-hospital mortality ( P < 0.001), hospital LOS ( P < 0.001), and 30-day mortality ( P < 0.001) improved. Conclusion Continuing QA program is effective in improving the clinical outcomes of hospitalized CAP.
