Treatment with Intravenous Methylprednisolone in Patients with Graves’ Orbitopathy Significantly Affects Adrenal Function: Assessment of Serum, Salivary Cortisol and Serum Dehydroepiandrosterone Sulfate

Abstract: Treatment of active, moderate-to-severe Graves’ orbitopathy (GO) is the administration of intravenous methylprednisolone (IVMP). IVMP may be followed by additional therapy with oral prednisone. The aim of this study was to analyze the impact of IVMP on adrenal function by evaluation of serum, salivary cortisol and serum dehydroepiandrosterone sulfate (DHEA-S). Fourteen patients received IVMP treatment (cumulative dose of 4.5 g in 12 weekly infusions) followed by oral prednisone (for three months). All patients… Show more

“…Intravenous methylprednisolone (IVMP) pulse therapy is currently considered as the first-line treatment for moderate-to-severe and active Graves’ orbitopathy (GO) according to the European Group on Graves’ Orbitopathy (EUGOGO) recommendations [ 1 ]. This therapy is associated with the risk of side effects including cardiac arrythmias, pulmonary embolism, elevation of liver enzymes, and suppression of the hypothalamic–pituitary–adrenal (HPA) axis [ 2 , 3 , 4 , 5 , 6 ].…”

“…Furthermore, Pelewicz et al [ 28 ] examined patients with orbitopathy, which is a consequence of Graves’ disease. The authors decided to check the impact of intravenous methylprednisolone (IVMP) treatment in a group that suffered from Graves’ orbitopathy.…”

“…Glucocorticoids (GCs) are highly effective and widely used for treatment of various autoimmune diseases. Therapy with GCs is associated with multiple side effects (1)(2)(3)(4)(5), including the negative impact on bone, leading to secondary osteoporosis and an increased risk of fractures (6,7).…”
