The current outbreak of viral pneumonia, caused by novel coronavirus SARS-CoV-2, is the focus of worldwide attention. The WHO declared the COVID-19 outbreak a pandemic event on Mar 12, 2020, and the number of confirmed cases is still on the rise worldwide. While most infected individuals only experience mild symptoms or may even be asymptomatic, some patients rapidly progress to severe acute respiratory failure with substantial mortality, making it imperative to develop an efficient treatment for severe SARS-CoV-2 pneumonia alongside supportive care. So far, the optimal treatment strategy for severe COVID-19 remains unknown. Intravenous immunoglobulin (IVIg) is a blood product pooled from healthy donors with high concentrations of immunoglobulin G (IgG) and has been used in patients with autoimmune and inflammatory diseases for more than 30 years. In this review, we aim to highlight the known mechanisms of immunomodulatory effects of high-dose IVIg therapy, the immunopathological hypothesis of viral pneumonia, and the clinical evidence of IVIg therapy in viral pneumonia. We then make cautious therapeutic inferences about high-dose IVIg therapy in treating severe COVID-19. These inferences may provide relevant and useful insights in order to aid treatment for COVID-19.

Despite the growing recognition of a potentially significant respiratory syncytial virus (RSV) disease burden in adults, relevant evidence in the United Kingdom (UK) is limited. This systematic literature review (SLR) aimed to identify the disease burden of RSV in UK adults, including certain high‐risk subgroups and existing evidence gaps. Published studies (2011 onwards) reporting epidemiological, economic and clinical burden outcomes in UK adults (≥15 years) with RSV were identified from indexed databases, including MEDLINE, Embase and the Cochrane library. High‐risk groups included elderly (≥65 years), immunocompromised, co‐morbid and co‐infected patients. Outcomes included RSV incidence/prevalence, mortality, clinical presentation and direct/indirect resource use/costs. Twenty‐eight publications on 28 unique studies were identified, mostly in general/respiratory indicator (n = 17), elderly (n = 10) and immunocompromised (n = 6) cohorts. Main outcomes reported in the general/respiratory indicator cohort were RSV infection incidence (seasonal/annual: 0.09–17.9%/6.6–15.1%), mortality (8,482 deaths/season) and direct resource use (including mean general practitioner [GP] episodes/season: 487,247). Seasonal/annual incidence was 14.6–26.5%/0.7–16% in high‐risk cohorts. Attributed to RSV in the elderly were 7,915 deaths/season and 175,070 mean GP episodes/season. Only two studies reported on co‐morbid cohorts. Clinical burden outcomes were only reported in general and immunocompromised patients, and no evidence was found in any cohort on indirect economic burden or RSV complications. Evidence captured suggests that RSV may have a substantial burden in UK adults. However, available data were limited and highly heterogenous, with further studies needed to characterise the burden of RSV in adults and to validate our findings.

RESUMO Objetivo: Construir protocolo de cuidados de enfermagem à criança no pós-transplante de células-tronco hematopoiéticas. Método: Pesquisa metodológica conduzida entre janeiro e setembro de 2019 e, em três etapas: a) realização de revisão integrativa (cuidados de enfermagem foram identificados); b) estruturação e organização teórica de cuidados a partir da Teoria das Necessidades Humanas Básicas e da Classificação Internacional para a Prática de Enfermagem, complementando os dados da revisão; c) desenvolvimento do protocolo, cuja elaboração integrou as etapas anteriores. A análise de compreensão de itens/cuidados do protocolo foi realizada por três enfermeiros especialistas. Resultados: O protocolo de cuidados é composto por 40 problemas de enfermagem e está organizado de acordo com as necessidades psicobiológicas, psicossociais e psicoespirituais de crianças submetidas ao transplante, integrando cuidados práticos e de orientação. Conclusão: O protocolo é opção tecnológica para a assistência de enfermagem à criança transplantada, com vistas à prevenção e manejo das complicações.
