Treatment results and complications after PFN osteosynthesis

Abstract: The PFN is an appropriate implant in cases of per-, inter- and subtrochanteric femur fractures. Anatomical resetting and correct implant positioning are the keys to successful osteosynthesis. The risk of implant failure is highest in the case of multi-fragmentary per- and intertrochanteric fractures in which medial strengthening has been degraded in patients aged over 80 years. The clinical results in elderly patients are unsatisfactory.

“…Although there is a wide range in the results given for ''quality of operative treatment'' between these studies (4.6-39%), it is noteworthy that comparable definition of this ''quality'' yields nearly identical figures: 20.7% in our study versus 20.8% in the study of Hohendorff et al [14] This does not release ourselves from the obligation to self-critically analyze and improve our operative results. It shows, however, that the technical problems described are commonly encountered and need to be taken into consideration when applying the PFN Ò .…”

“…In 10 of these 47 cases, mechanical complications occurred. Hohendorff et al [14] analyzed results and complications of 139 PFN Ò osteosyntheses retrospectively and found a 31.7% complication rate. Their total complication rate was higher mainly because nonmechanical complications like for example hematoma and heart failure were included.…”

“…In patients with no general complications, the 3-month mortality rate was 2.4%. If a patient had two complications, the 3-month mortality rate increased to 32% to 8% of cut-outs were reported [1,14,[23][24][25]. Five patients (1.2%) experienced lateral blade slide with irritation of the tractus iliotibialis.…”

“…Higher revision rates after PFN-osteosynthesis are reported. These range from 7.1 up to 27.3% [1,14]. The frequency of femur shaft fractures following gamma nail treatment is recorded at 2.9-5.6% by various authors [2,3,7,8].…”
